1 Cake q3 BARRIERS Understanding the barriers we use every day
CAKE Q3 addresses barriers. In previous years we have focused on barriers against major accidents, but this year, in light of the HSE trend we have seen for MDN, we want to focus on the barriers that are important to prevent the types of incidents that we have seen on the rigs in Q1 and Q2. Hence, we will focus on the barriers that we use every day in order to prevent occupational accidents. Barriers can be technical – the measures and equipment that we have in place to safeguard us during operations and maintenance Barriers can also be oraganizational – such as our procedures, our work execution system, TBT’s etc, that help us identify risks and determine how to mitigate them.

Discuss in meeting Tips, ask people to relate barriers to a job they are planning or have already done. Hint about which barriers to ask about: TBT, WP, procedures, training, PPE, supervision, isolation, communication, physical barriers, checklists

INCIDENT LTI Maersk Intrepid , Injury to hand during maintenance job on Samson vacuum unit Barriers - to PREVENT incidents from happening Barriers - to LIMIT consequences from the incident Procedures TBT Emergency preparedness organization Communication Training PPE Isolation Personal risk understanding The image above shows some of the important barriers that were found to be inadequate in the incident investigations (marked in red) Incident Mærsk Intrepid, : The Maintenance Engineer (ME) and the engine mechanic onboard the Maersk Intrepid was function testing the Samson vacuum unit. The discharge valve is tested in manual mode, and is working, but when switched to automatic mode it will not open. The ME tried to free the valve by hand by gripping the valveshaft. Suddenly the valve opened and closed, which resulted the ME getting his hand squeezed between the end of the valve gate and the valvehousing. Barriers found failing/ missing: No planning and risk assessment/TBT Non-conformance to WEP process (no isolation of equipment) Lack of supervision (M. Eng. New to rig) Miscommunication between night and day shift regarding need to work on vacuum unit. Supervision Barriers don’t always work as intended

5 Findings from investigations of accidents in MDN in 2016 show that risk understanding during work execution is a barrier that we need to improve. HOW CAN WE WORK TO BUILD AWARENESS OF RISK DURING WORK EXECUTION ? CAKE Q3 ACTIVITY: Stop the job during execution of a work tasks, both routine and non-routine. Take time to consider how we are using the plan from the TBT. Aim of the activity: Increase awareness about use of TBT during work execution, and the importance of continuous assessment of risk during jobs. Participants: All employees on the rig Time to be spent: 5 minutes once a week – during one job – during more if you wish and have the time Content: Once per week, employees are to pause work during a job to consider: - Have we identified all risks in the TBT, or has something come up along the way? - Are we adhering to all the precautions we identified during the TBT? - Has the job changed? Tools: Card with questions related to stop the job to be distributed to all employees by section leaders Follow up / reporting: Section leaders are to ensure that employees carry out the activity. Synergi reports are not required.
