All 258 passengers and 13 crew on board were killed, along with two people on the ground The pylon, the rigging holding the engine onto the wing, had been damaged during an AA maintenance procedure. (amended) 2002/09/01 (2) The training program shall include: (a) initial training to ensure that persons performing elementary work or servicing are aware of the pertinent regulations, standards and air operator procedures associated with that work;
(amended 2002/09/01; (b) update training to ensure that personnel remain competent and are made aware of any changes to the pertinent regulations, standards and air operator procedures;
(amended 2002/09/01; (c) additional training where it is shown to be necessary by a finding made under the evaluation program maintained under section 706.07 (d) procedures to ensure staff are kept aware of maintenance safety related issues in general, by means of bulletin boards, information notices, company publications, verbal briefings, or by similar means.

Errors tend to occur when negative emotions patterns cause us to develop rigid thinking patterns.
