As case manager, it is your responsibility to know about agency services AND invite the agencies to the students’ T/IEP conferences.

This MUST start with the 703 form This MUST start with the 703 form. The 703, when signed by the parent (or a student 18 and older), allows you to invite an agency or agencies to assist with post-school planning.

Here are a few of the services that an agency might be able to provide…

Medical and psychological assessment

Vocational evaluation and planning

Career counseling and guidance

Training and education after high school

Job-site assessment and accommodations

Job placement and job coaching

Supported employment

Vocational and other training services (tuition, books, supplies, fees)

Some agencies include: Division of Vocational Rehabilitation Agency for Persons with Disabilities Deaf and Hard of Hearing Services of Florida Division of Blind Services Lighthouse for the Blind Pasco-Hernando Community College

If you decide that it is too much extra time and effort to send the 703, or address the needs of agency involvement, be sure to have something else on hand…

A Money Tree!!!

Decisions about the need for agency involvement should not be made by you alone. This decision can be costly to the student and possibly to you. Empower yourself with more information. ALWAYS do a 703 for students that will turn 16 (15 and older) and beyond.

Transition is not JUST about getting students ready for graduation….
