Outreach 3  Spread the message and reach those who need your program the most  Make sure applicants are suitable for the program  Adapt your plan as needs in your community shift Follow-up Eligibility Exit Program Services Outreach Intake & Assessment

Eligibility 4 Member of a federally recognized tribe, an Alaska Native, a Native Hawaiian, or an Indian as determined by a policy of the grantee Unemployed, under-employed or low income Registered for the Selective Services for males Follow-up Eligibility Exit Program Services Outreach Intake & Assessment

Intake & Assessment 5  Assess suitability and determine eligibility prior to program enrollment.  Develop the IEP through comprehensive consultation with participant. Follow-up Eligibility Exit Program Services Outreach Intake & Assessment

Case Management 6  Case managers help participants access services that will assist them in attaining the skills needed to secure and retain unsubsidized employment.  Services may vary from grantee to grantee and between tribal programs and non-profit organizations.

Exit 12 Exit usually occurs when participants have attained unsubsidized employment and are stable in their new jobs. Exit occurs when a participant does not receive a service funded by the program or a partner program for 90 consecutive calendar days and he/she is not scheduled for any other future service.
