 Within 3 weeks of ‘hospital discharge letter received’ -post emergency admission - £120: ◦ Review ◦ Medication reconciliation  Consent for LTC patients - £10  Internal practice review monthly (emergency admissions and A&E attendances)  Timely telephone access via an ex- directory or by-pass number to ambulance staff and A&E clinicians & care homes & other care providers, (e.g. mental health and social care teams)

 Moving from activity to targets  IC Level 2 (ex Coordinated Care) is now called IC Complex  Complex comprises: ◦ Mandatory (dementia, palliative care, nursing home) ◦ Discretionary (clinical decision, new register, the code to use is 9NgzB=eligible for integrated care, with a date from 1st Apr 2016 onwards)
