4 C&P Care Home Group Overview Multiagency membership Countywide coordinated approach to: – Address issues – Promote developments – Pilot new initiatives – Gather intelligence & data Aims – Improving the Quality of Care delivered by Nursing and Care Homes – Improving the integration of Care homes with the local health system – Monitoring quality at strategic and system level – Provide support / intervene where appropriate – Reducing attendances and admissions to/at hospitals from care homes – Develop an effective single CCG wide discharge protocol for Care Homes
