How do I access the disclosure form? Go to Office of Research Compliance website Click on the eCOI link under Conflict of Interest. Click on the link provided ( If unable to access disclosure form – go to your browser history and delete the history.

My College/Department Name is not correct Click on the “User Management” tool link. Scroll down to Department & click on drop down arrow. Click on the correct Department Name. Click on “Save Profile” at the lower right corner.

My Spouse is not in the drop down list. Click on the link “ them” and your will appear with instructions. Remove your name from the “To” line and enter in your spouses . Keep the “user registration” link in the body of the . Send the to your spouse so that s/he can register name in the coi database. Once your spouse has completed the registration, you will need to log back onto the coi database to complete and submit your form.

The company name is not in the drop down list? Enter the full name of the company and it will be added to the list of company names. Continue with entering in the requested information. Click “Save & Continue” at the lower right of page.

How do I edit or remove a company off my form? Click on the company name that needs to be removed. Click on the “X” icon.

How do I delete travel information from my form? Log onto the COI database ( Click on the “Travel” tab on the left side of the screen. Locate the company name that needs to be deleted. Click on the “X” icon.

How do I review the information on my form? Log onto the COI database ( Scroll down the page to review the information.

How do I add /make changes to my disclosure form? Log onto the COI database ( Click on the “Modify Disclosure” link. Look at the left side of the screen tabs (Key Responsibility, Companies & Travel). Click on the section that needs to be updated. Click on the “Pencil” icon and make changes/additions as needed. Click on the Overview & Confirmation tab to review, check box to verify information, & click on “Submit Disclosure.”

How do I get a copy of my disclosure form? Log onto the COI database ( Click on Download PDF version.

How do I get a copy of previous years disclosure forms? Log onto the COI Database ( Click on the “Re-winding Clock” icon on the top left- side of the screen. You will be able to review all forms that were submitted from 2012 to 2015 reporting years.
