13 Virtua qualifies patients by querying Amalga (e.g., future surgery patients) and generates an mailing list is sent to patients with a hyperlink The hyperlink brings the patient to a specific form in a SharePoint document/forms library after LiveID authentication. Forms are web- based. Patient completes the form and hits Enter. Forms data (structured) is saved in Amalga for use by clinicians (a new “view”) and in the patient’s HealthVault account Patient data always remains secure through the transaction Secure Patient Forms How it works:

15 While structured data in PHRs is a long-term industry goal, it will take 5 – 10 years before most private physicians are using structured-data EMRs, and even then there will be residual paper. Zweena provides services and technology to store and present scanned paper medical records, via the Web. Zweena also abstracts data from the paper record to create metadata for patient use, and it has various consumer tools (e.g., health risk assessments) Patients use Zweena via a user account, maintained by Zweena Zweena’s database is synchronized with HealthVault LiveID is Zweena’s user authentication method Scanned Paper Records What and Why?

16 Scanned Paper Records How it works... Zweena will be an option presented on Virtua’s PHR “jump-off” website page Like other third-party applications, the patient will need to link Zweena with HealthVault and establish a Zweena account Zweena works directly with the patient for the patient’s authorization to obtain copies of medical records from physician offices (usually via facsimile) Zweena scans the records into Zweena’s data base Records are stored in Zweena’s data base and can be synchronized with HealthVault. Experiment: Virtua will subsidize patient’s cost for one year.
