3 Getting Started Go to www.google.comwww.google.com Click on Documents in menu across the top of the page Sign in with your username and password Click on the red create button in the top left corner of the page Select Form

4 Creating an Assessment Give your Google doc a title and provide instructions for your students For each question select a question type (text, paragraph text, multiple choice, check- boxes, scale, grid

5 Text Question Entry Make sure to click on “Make this a required question” to ensure that all questions get answered

6 Moving Forward After you hit “Done” for the first questions the screen will appear like this. Click on “Add Item” to enter another question

7 Multiple Choice Questions

8 Editing or Duplicating Questions Once you have hit “Done” for any question you can go back and edit a question or duplicate it. Put the cursor over your question and the ions on the left will appear.

9 Multiple Choice, Checkboxes, and List Multiple Choice and List only allow for one answer. Multiple Choice allows the examinee to see all answers at once. List stores all answers in a drop down menu Checkboxes allows the examinee to select more than one answer.

10 Scale Questions

11 Grid Questions If you do something like this then you cannot make it a required questions. Otherwise you would need an answer for each row.

12 Changes at a Later Date Access the form after logging onto Google Docs Click on document Click on Form and then click on edit form

13 Analyze Data When you open up Google Docs and click on your form, the data will appear in spread sheet form. You can sort the results by clicking on “Data”
