16. There are four pages of Form 10, Certified Assurances. Signature of authorized representative on p. 4.

19. There are four pages of Form 13, Disclosure Report. Follow directions for pages requiring signature of authorized representative.

20. Additional Forms are needed with the application: 1.SAMS Registration/DUNs # 2.Civil Rights Certification (put N/A on line for Grant #)  Note: The Environmental Forms now have fields that you can type in. Check out website.
