Educating the Staff  While reviewing the requisition form with the staff, highlight their initial page on the requisition the pertinent information so the staff can use it as a guide  Instruct them to peel off the security strip on bottom of the requisition form, already filled out and place over specimen

Completing the Process Place the POC or PLAIN specimen cup inside the specimen bag provided by Principle Labs, LLC., and seal the bag Separate the requisition forms, fold the top white laboratory copy with a copy of the patient’s demographic sheet and place in the outside pocket of the specimen bag.
