Options for Production Reporting Hardcopy submission –Fill in official form which is available from Forms page link –Postmarked by 3/31 –Form preprinted by DEP with available information pre- populated when address correct in our system. –by early March to: smaller operators with 1 well – 1,300 operators 2-20 wells – 1,900 operators wells – 400 operators > 110 wells operators - reminder letter only If no letter received by early March – DEP may not have good address. Call Ruth Plant at to verify address. OR File Electronically via web site (preferred)

Well Records - 1 & Completion Reports Pursuant to Section 212(b) of the Oil and Gas Act and Section (a) of Chapter 78 of the Oil and Gas Regulations, a Well Record must be submitted to the Department within thirty (30) days of cessation of drilling or altering a well. 2.Pursuant to Section 212(b) of the Oil and Gas Act and Section (b) of Chapter 78 of the Oil and Gas Regulations, a Completion Report must be submitted to the Department within thirty (30) days of completion of the well.
