10 Important Safety Reminder Treatment for a lab- or work-related injury Even if you think the injury is too minor to do so, you must report the injury to your supervisor who will assist you in seeking medical treatment. When seeking treatment remember to bring MSDS &/or other safety info with you. Please notify medical staff that the treatment is due to a workplace injury or illness, so that they can initiate the proper paper work. 13 219 MacKay Fire, 8-18-10 Reminders:  Perform annual online fire safety training  Inspect all power cords for fraying or damage,  Plug heavy duty equipment right into power outlet.  Keep lab area uncluttered. Helpful advice:  Keep lab notebooks in water tight containers,  Have your gas shut off if you are not using it.

14  Chemical Safety Board (CSB)  Conducts root cause investigations of chemical accidents at fixed industrial facilities  Will start investigating academic laboratory accidents starting Sept 30, 10.  ISU community could become subject to new rules.  Important because if we have any serious accidents they could become part of the CSB investigation.  Will raise a spotlight on colleges and there is the potential for more regulation.  Standard operating procedures should be developed, and training documented for, procedures that involve hazards New Information

15 If you have never filled out a Hazard Inventory form or it has been over 2 years since you have filled out a form, you will need to complete a new form. The animal section was revised on the Hazard Inventory form in October 2008 for AAALAC accreditation. Anyone working with animals needs to make sure the hazards are current. Please complete and sign the attached Hazard Inventory form. Have your supervisor review and sign the form, then return the completed form to Environmental Health & Safety, 2809 Daley Drive / 3660 as soon as possible. If your work environment does not include any of the hazards listed on the Hazard inventory form, please complete the personnel information section and note in the comments section that you do not work with or are not exposed to any of the listed hazards.Hazard Inventory form After your completed Hazard Inventory form is received by Environmental Health & Safety, you and your supervisor will be contacted regarding any required medical surveillance. If no medical surveillance is required, you will receive no further communications regarding the Occupational Medicine program. New Information
