6 Patient Engagement How we developed our Virtual Patient Participation Group Would you like to take part in our “Virtual Patient Reference Group ?” The Patient Reference/Participation Group is an email community, which allows the Practice to correspond with patients on a regular basis about the services that we provide to you at our surgery. If you are interested, require more information or wish to join and become part of our “virtual patient reference group please contact Mrs Johanne Shorrock, our Practice Manager by email on: johanne.shorrock@nhs.net

8 Patient Engagement Hello We are encouraging patients to give their views about how the practice is doing. We would like to be able to find out the opinions of as many patients as possible and are asking if people would like to provide their email addresses so we can contact you by email every now and again to ask you a question or two. Are you interested in leaving your email details? If you could fill in this quick form and hand it back to reception (or provide your details over the phone to me) we will add your email address to a contact list. Your contact details will only be used for this purpose and will be kept safely.

10 Patient Engagement Consider your patient age groups – Consider survey data re: catchment (doctor foster) age, jobs, wealth, families, transport, technology etc – What’s available in your area – How easily is it accessible – What the transport service like etc – What IT system do you use – does it have functionality that enables other options of engagement

11 Patient Engagement Try to keep ahead of the game Scan the horizon for new options Check what is available and what might be becoming available Network across your patch and further afield Try to be flexible and adapt to patients needs
