4 Current Process Students are evaluated quarterly by employers for co-op assignments Evaluation forms are mailed to employers Employers must mail completed forms back to the co-op office Some forms are manually scanned and analyzed

5 Problems with Current Process Paper and pencil evaluation process is inefficient Standard mail can be slow, costly, and unreliable Need more automated reporting capabilities Need better tracking of evaluations and contacting employers

6 Proposed Solution Web-based system Co-op office can notify employers by email to submit evaluations Online form submission  Eliminates postage costs  Tracks evaluations Increased reporting capabilities for both the co- op office and academic departments

7 Major Features Employers can fill out evaluation forms online Co-op office and academic departments:  Can search for & view completed evaluations  Can run reports on completed evaluations  Update the evaluation form Co-op office:  Email reminders to employers  Export data to Excel

8 User Stories Employer submits an evaluation User views an individual evaluation User creates a report Co-op representative sends a reminder message

9 Employer Submits an Evaluation Once an employer is logged on, an evaluation form is displayed. The employer fills out the form to evaluate the student. Once finished with the form, the employer submits the information. The information is verified and a printable version of the evaluation is displayed.

10 User views an individual evaluation Once logged on, the co-op or academic representative clicks “View Student Evaluation.” A new page loads asking for student identification. Once entered, all the evaluations for that student are listed. The representative selects one to view and a copy of the form is displayed on the screen.

11 User creates a report Once logged on, the co-op or academic representative clicks “Create Report.” A page loads asking for the criteria to run the report on. The representative fills out the desired information and submits the request. A page loads with the results of the report printed on the screen.

12 Co-op representative sends a reminder message The representative clicks “Email Reminders.” A page is displayed with the link “Send Reminders.” The representative clicks the link and the emails are scheduled to be sent to all employers that have not yet filled out evaluations.
