2 Did you know that you can view all of your Personal Information that we have on file for you in Employee Self Service? The Personal Information menu enables users to: Change your password and security question for Self Service access View the current address and email we have on file for you View and update your Emergency Contact information View and update your Campus Emergency Alert Contact information

7 Did you know that you can view all of your Benefits and Deductions on Self Service? The Benefits and Deductions menu enables users to view: Benefits Deductions Beneficiaries and Dependents Personal Benefit Summary Please note: addresses, phones and email can only be viewed for accuracy here. Although it may seem like the easiest place to make changes using BannerWeb, due to data integrity and address hierarchy standards, addresses still need to be changed through the Human Resources office. The Personnel Data form is available on the View Addresses page for you to complete and forward to Human Resources to make any necessary corrections.

The Benefits and Deductions menu enables users to view: Benefits Deductions Beneficiaries and Dependents Personal Benefit Summary We need your help to update your Emergency Contact Information. Click on the View or Update Emergency Contacts Link to view the current contact information available or add an Emergency Contact if you do not have one on file. Don’t forget to SUBMIT CHANGES button when you are done!

13 Benefits and Deductions are grouped together; click on a link and it will open a new page showing your current (and past) deductions in that group.

14 -your individual plan description -the % you are contributing -the amount you are contributing -the maximum amount you can contribute Links at the bottom of each deduction will take you to: History - lists effective dates of changes to your individual deduction Contributions or Deductions – list of deductions by pay-period Vendor Web Site – takes you to Web Page of Vendor or Summary Plan Documents for your Plan This will be used when we do open enrollment but is not functional yet

15 Clicking on the Benefit Statement allows you to produce a complete Summary of all your benefits and deductions as of a specific date; showing who is covered, who your beneficiaries are, what plan you are enrolled in and both the employee and employer share of the cost.

18 Did you know that you can view your Pay Information as well? The Benefits and Deductions menu enables users to view: Benefits Deductions Beneficiaries and Dependents A personal Benefit Summary To delete or change information of an existing dependent or beneficiary, click on “their name”; another screen appears for you to edit the current information or delete the record. Add a new dependent or beneficiary by selecting the Add A New Person link and entering their personal information and SUBMIT CHANGES. Clicking on the Coverage and Allocations link will show you who we have on file as being covered under a benefit (Health and Life Insurance Plans) and who is/are beneficiary(ies) for a plan (Retirement and Life Insurance Plans). The allocation section shows how the benefit is to be divided up among the beneficiaries and whether they are a primary or a contingent. Information entered on BannerWeb was initially entered several years ago when we changed Life Insurance carriers based on forms we received at the time. Please review what we have on file and make any necessary changes so that your information is current and correct.

19 Please note: the Start Date Column for many deductions is 2004 because that is when we began to implement Banner. If you were hired after that or started a benefit or deduction after 2004, the start date should be a true reflection of the effective date. If a deduction is set up on an annual basis, an end date will show in the Stop Date Column. Clicking on the Coverage Details and Beneficiary Allocation Details links allows you to make changes to the dependents or beneficiaries you have listed for your benefits.

21 PAY INFORMATION Direct Deposit Allocation – shows exactly how much and to where the earnings from your last paycheck was deposited. Earnings History – allows you to see how much you have earned by earn type between a begin date and an end date of your choice Pay Stub – a list of every pay-period’s pay-check; complete with all the information you receive on a paper pay-stub or deposit slip. There is a printer friendly button available for you to print out yourself if you need a copy of your pay stub for any reason. Deductions History – similar to the Earnings History; allows you to see how much you have had deducted by deduction code between a begin date and an end date of your choice Earnings by Position – if you have held more than one position at Alfred University, this shows you the earnings associate with each one.

22 TAX FORMS: W4 Information – shows the Status and Allowances you are currently claiming on your Federal W4 form and the New York State IT-2104 form W2 Form – displays the information contained in your W2 forms for tax years 2005 through present. There is also a Printer Friendly button to use to print the form in an approved format suitable for filing your tax return
