1 Quality Education for a Healthier Scotland Appraisal Forms and PDP for Appraisees (Primary and Secondary Care)

3 Appraisal Period – an explanation Interview details on SOAR include names of Appraiser, Appraisee, Date & Time and Location of meeting(s) – as well as “Appraisal Period” Appraisal Period – counting in fiscal years – is used to log WHEN the appraisal took place, rather than WHAT materials were discussed For example, an appraisal that took place on 18 th March 2014 would fall under 2013/2014 It is expected at the annual appraisal that appraisees will discuss materials within last 12 months / since last appraisal This is to satisfy Revalidation requirements that the appraisee is being annually appraised

5 Personal Development Plan (PDP) (1/2) Prior to your appraisal, you are asked to reflect on your PDP, and reflect on what learning requirements you might have for the year ahead This is to be logged on Form 3 Domain 1 Although GPs and Secondary Care users have different forms for this, the process is the same: – Review PDP, document additional learning – Propose new PDP / learning for year ahead

6 Personal Development Plan (PDP) (2/2) When your appraisal forms are submitted, your draft learning items will pre-populate Form 4’s PDP section The Appraiser can amend the PDP according to your appraisal discussions When you sign off Form 4, the finalised learning items will act as your PDP for the year ahead, and this is transferred to your Form 3 PDP section accordingly

7 Appraisal Forms – at the end of the appraisal When you have approved Form 4, all submitted forms and documents are archived to that appraisal; and Your draft Form 3 is wiped/reset to allow for preparation of next appraisal – PDP from Form 4 will be transferred to Form 3 Forms 1 (Background) and 2 (Job You Do) are not reset. You only need to edit details if there are any changes.

8 Form 4 PDP section is populated with draft PDP on Form 3 When Form 4 is signed off, learning items on section C are transferred to Form 3 and become PDP for year ahead PDP will only appear if using appraisal forms on SOAR (i.e. we can’t import from spreadsheet or Word files, each learning need to be copied/pasted into SOAR individually)
