2 Honeywell Confidential – Do not forward or distribute 2 LifeStream Manager: Utilizing Reports Prove an understanding of the three standard report types and how to use them to enhance your TeleMonitor program and manage patients more effectively. Be able to export and share these reports as needed. Goal Know the basics of how to use LifeStream reports as an effective management application. Recommend key actions and steps to effectively utilize these reports. Objectives Module Length: About 60 minutes Best Practice: Access to the LifeStream Manager application is recommended if completing training as a self-study, or in conjunction with Clinical Consultant training. Pre-requisite: Designed for users with 6 weeks or more of data in their LifeStream application, after completion of LifeStream Basics and Intermediate Training. Understand the basic reporting functions in LifeStream. Analyze key metrics and generate evidence to determine if Key Performance Indicators have been achieved and maintained. Knowledge of how reports can be generated to: Quantify and measure the value of TeleHealth. Identify metrics or processes that need attention. Be able to export reports for storage and review. 5 Honeywell Confidential – Do not forward or distribute 5 Before You Start For all reports, always click Report Options in the menu bar above your report screen in order to customize the data presented. Different reports may be displayed by selecting Sites/Categories drop down: Underlined columns may be sorted by clicking once or twice on the heading.

6 Honeywell Confidential – Do not forward or distribute 6 Sharing Reports All standard reports may be shared in many ways with just a few simple clicks. Click to print via the regular print settings window. Instant one-click printing will print the report displayed on the user’s screen. Click to fax a report. Save a report in a variety of file formats, (shown on the right). If exporting as a graphic document, you will have the ability to choose a graphic file format and the desired picture resolution. You may also send a report via email, directly from LifeStream. You may choose a variety of export options (seen below). You will also be able to choose where to save the document. This menu bar is displayed above any report.

7 Honeywell Confidential – Do not forward or distribute 7 Sharing Reports: Best Practices Always get permission to share reports with other care providers and patients. Ensure the recipient is aware of what report is being sent. Always follow privacy policies when transmitting patient information. Follow-up directly with the recipient to interpret the information properly. Save files in a consistent and universal file format, per your organization’s data collection policies. If you are unsure about file formatting, contact your administrator. Sharing is a powerful tool, when it is done properly. Here are a few best practices when regularly working with reports:

In this case, we see the monitor configuration was overwritten by someone. The program manager can discover who made changes and why. Discover the programmed questions for a patient’s monitor and technical info to troubleshoot problems. Patient Reports >

Graphical Trends Report Reports can be customized to display a variety of time frames, vital signs and device test results. Go to the “Report Options” icon (arrow #1) to get the settings screen. After you select the “Report Options” icon, a screen (arrow #2), allows you to adjust your patient report with a variety of inputs. 1 2 Patient Reports >

Multi-Patient Trends Report Patient Reports > Can be displayed in graphic or tabular format and customized Print/fax all of the patients for one physician or case manager in one step.

Select the items to include. 4. Group patients by their Sites/Categories, such as: Physician; Care Provider or Diagnosis The information displayed depends on the report options that are selected. After you select the “Report Options” icon, customize the options as needed. Patient Reports >

Notes Report Use as needed during triage or clinical review for patient management. Useful to bring up an at-a-glance recap for a selected patient. Reports will be organized into sections like this for easy reference: Patient Reports >

Patient Activity Report Run weekly to review clinician productivity and management of TeleHealth patients. Report Options include adjusting the Report Period: Patient Reports >

15 Honeywell Confidential – Do not forward or distribute 15 Presents patient information such as: Alert Limits; Medications; Diagnoses; Contacts; Care Providers and Insurers. Print out as hard copy of patient information as required. This report is often used in accordance with emergency preparedness plans. Patient Information Report Patient Reports >

To review triage responses after an emergent care event. As supportive evidence for continued services To coordinate triage responses and patient education to overall plan of care Run monthly and bi-monthly to improve patient management and clinical efficiency.

In the event of an infectious disease outbreak, this report gives you the ability to retrace which patients used the monitor. View equipment history by patient or by serial number. Pull report by patient name or serial number and select the time period. Manage equipment and inventory effectively with a full history report. Site Reports>

22 Honeywell Confidential – Do not forward or distribute 22 Track number of monitor units assigned and calculate the percentage of units in use to determine the amount of patients currently under TeleHealth monitoring. Sort by all equipment or sort by a particular type of equipment. Keep a concise record of units, serial numbers and notes on all of the equipment. Use the Sites/Categories drop down menu in the upper right corner for smaller data sets. Equipment List

Review the percentage of patient compliance within a defined period of time. Report Options include Unscheduled Readings as well. Analyze Analyze the percentage of patient compliance and compare. Reallocate resources if necessary. Determine patient parameters settings. Improve clinical efficiency. Note patient instability and adjust care plan based on monitor readings. Determine amount of patient knowledge about equipment and plan patient education.

Analyze the length of service on TeleHealth and Key Performance Indicators. Note the correlation between length of TeleHealth service and outcomes.

31 Honeywell Confidential – Do not forward or distribute 31 Multi-Patient Response to Vitals Options After selecting Report Options, customize the report by first selecting the Diagnosis and the Report Period. The Responses to Include and Response Details must also be chosen. Once you select a diagnosis, click the icon (indicated by the arrow on the left,) to add a Diagnosis to the list. A single arrow adds one Diagnosis to the List Click the double arrow icons to add or remove the entire Diagnosis list.

32 Honeywell Confidential – Do not forward or distribute 32 Multi-Patient Response to Vitals Report This report display (left) numbers all of the patients in the system with a selected diagnosis. Use regularly to review the responses to vital signs in order to improve patient management and clinical efficiency. Review patient management like the amount of phone calls to a physician, unscheduled clinical visits, patient education and more. Review triage responses after an event. Coordinate triage responses and improve patient awareness of the overall care plan. Program manager can pull the report in order to get a quick idea of how the clinicians are responding to the data and then provide feedback to the clinicians on charting.

33 Honeywell Confidential – Do not forward or distribute 33 Allows the LifeStream user to track and monitor all outcomes questions, messages, reminders, and/or requests for a patient to retest during a specified time period. This also tracks which clinician sent a message, who acknowledged it and whether the patient complied. The monitor must be compatible with the patient messages feature (Genesis DM Version 4.12.11A and above). The customer PIN must have the “Patient Messages” option enabled. The LifeStream user must be signed into LifeStream as a Clinician, Clinician/ Administrator or Administrator. Patient Messaging Report Patient Messaging Improves the Quality of Care! Patient messaging is essential when an agency does not have the resources to place phone calls to patients on a routine basis. Messaging may answer non-emergency questions, educate customers and provide personalized reminders! Site Reports>

Administrators may provide reassurance to interested parties that an audit trail is securely produced. Use to understand who performed any actions related to a patient. Document for compliance purposes. Can be easily printed and exported for data collection. Organization Reports >

41 Honeywell Confidential – Do not forward or distribute 41 In addition to the Responses and Comments fields, the Outcomes Questions allow for the clinical to provide additional information about the “Yes” response(s) to questions number 48- 51 1. Responses: In this instance, the clinician selected Changed Medication and Visited Emergency Department. Note: If the patient answers Yes to one or more questions, the clinician must provide a response to the follow up questions listed in the Outcomes Questionnaires field before acknowledging the reading. 2. Outcomes Questionnaire: Because the patient answered Yes to question 48 and 50, the clinician is prompted to select a cause for the ER visit. The clinician will scroll down to respond to the additional prompt associated with question 50. 3. Managers may run this report monthly to determine how active the program has been. Organization Reports >

This is important to identify and correct all errors listed. Be sure to check all of the error boxes. Any monitors listed here have data coming in to LifeStream but are not assigned to a patient.
