4 Program Guidelines SSN to EIN conversion are only available for existing Solavei Members SMO and SMS member types, not applicable to MEM In addition, as a minor cannot be a Social Member a minor cannot convert to an EIN. Conversion to EIN is only available to Single-Solavei-Member LLCs A Solavei Social Member may only have a total of 6 accounts. This can be a combination of EIN’s and SSN’s but may not exceed 6 SSN to EIN Conversions will be submitted monthly: Ongoing updates will be submitted on the 1 st business day of the each month. Solavei Compensation Payments may only be made for a checking account using a direct deposit method for accounts with EIN’s. The Solavei PayCard will remain open, unless the member chooses to close it with Money Network Confidential and Proprietary |4

5 Documentation Requirements and Validation The following documents are required from the member to complete the SSN to Single- Member LLC EIN Conversion Complete Solavei Single-Member LLC EIN Conversion form (Found in Support Content) Complete W9 tax form per instructions on the IRS form (Found in Support Content) Copy of Articles of Incorporation/Formation/Organization or Operating Agreement (unique by state to be supplied by member) Voided/cancelled check with the Business Name OR Individual's name, address, & bank routing information (unique by bank to be supplied by member) Confidential and Proprietary |5

6 Document Verification Member is required to submit all documents together MSS is to validate the name appears consistently on all documents, MSS should validate: Name is consistent on: Member Account, Solavei Form, W9, Articles of Incorporation/Formation/Organization or Operating Agreement Note: Articles of Incorporation may have multiple names. The member’s name must be listed as a Director, Incorporator, Registered Agent or Business Owner Verify the Business Name (Single-Member LLC) is consistent on all documents. Voided checks may be in the Members name or the Business Name only. If documents are incomplete (not submitted, not filled out, or names do not match) email the member from the SalesForce case and request updated documents be resubmitted. The member should resubmit all documents together- even if some were complete. Confidential and Proprietary |6

8 New Requests (E-mail and E-Fax) 1.Access the EIN mailbox via portal.microsoft.com Login at https://portal.microsoftonline.comhttps://portal.microsoftonline.com Click on Outlook from the top of the window Click the arrow just to the right of your name and choose Open Other Mailbox Type in EIN and click Open 2.Review inbox for new email requests 3.Select the oldest request in the Inbox, and flag the item using your chosen flag color 4.Open a case in SalesForce using the “EIN” queue: subject “SSN to Single-Member LLC EIN conversion” 5.Review the documents for completeness and ensure Member Name and Business Name are consistent on documents Confidential and Proprietary |8 All requests will be submitted to the EIN@Solavei.com mailbox in writing via the e-fax or scanned and attached in email. Verbal requests from member calls are not accepted (no Tier III tickets). 10 New Requests Cont’d 1.If documents are incomplete, missing or names do not match: Email the member via SalesForce, using the applicable template, to request updated documentation be resubmitted (never respond from the EIN mailbox- responses will appear as your name) Forward the members original email to EIN@solavei.com changing the subject line to: Member ID, SalesForce case number, Estimated Submission Date, Your Initials, Status as Incomplete.EIN@solavei.com Example: 54111, RN12011-124969, 01/07/13, JH, Incomplete. Move the email to the Incomplete folder. Leave the case in SalesForce as open and document reasons resubmission is required. Check open cases vs new cases to the mailbox daily. Close the case after 3 days if no response is received. 2.If documents are complete: Email the member via SalesForce, using the applicable template, that the request has been reviewed and submitted for processing. Forward the members original email to EIN@solavei.com changing the subject line to: Member ID, SalesForce case number, Estimated Submission Date, Your Initials, Status as Incomplete.EIN@solavei.com Example: 54111, RN12011-124969, 01/07/13, JH, Complete. Place the case in SalesForce as “pending” and document that it has been submitted. Move the email to the Submitted folder for the appropriate month (eg: Submitted 6/1) Confidential and Proprietary |10

12 Updating Solavei Systems 1.Open Enabil.  Search by full 10 digit member ID  Under “Summary tab”, click on “Customer Information” then add the “Business Name” in both places (SMS) or in 1 place (SMO) 2.Open Jenkon.  Search by full 10 digit member ID  Under “Menu”, click on “Account Services”, click on “Account Information” then “Primary”.  Click edit button and update the “Company Name field” with the business name & update 9 digit EIN number in the “Tax ID” field. Press SAVE.  Click “Banking tab” and update all fields  Under “Menu”, click on “Account Services”, click on “Custom Plan Settings” then click the “edit button”. Update the “ACHFeeFlag” to “yes”. Press SAVE Confidential and Proprietary |12 Solavei will complete the update from SSN to Single-Member LLC EIN

13 Completed Requests 1.Once batch process is complete (example: June 8 th 2013), Solavei Team will update the Sharepoint with requests completed by changing the status from Open to Complete. Pending Status would be used in the rare occasion that an error occurred in processing the request and the team needs additional time to complete. 2.We will send an email to MSS EIN team once completed to begin audit and then send completed emails. 3.MSS completes an audit of SalesForce and Customer Center to ensure: The Business Name and Member Name appear as the account name (Customer Center) The last 4 of the SSN field is now populated with the last 4 of the EIN (SalesForce) 4.MSS sends a final confirmation email from SalesForce to the Member confirming the change has occurred. Then “close” the SalesForce case after you send the email. Confidential and Proprietary |13 Solavei will complete the update from SSN to Single-Member LLC EIN in a monthly batch file. Systems updated include: (Compensation), (MySolavei), (Customer Center) and (SalesForce)
