14 Progress Markers for System Change Desired System Change : Portland’s Comprehensive Plan includes goals, objectives, and measures that address health determinants and impacts to create complete communities that are equitably distributed throughout the city. Expect to see: Initial, minimum, easier changes 1 At least one objective around key social determinants of health: equity, education, economic opportunity, social connections, active transportation, and food systems 2 Compelling justification of why these social determinants contribute to the vitality of Portlanders Like to see: Challenging changes 3 Integration of social determinants of health goals throughout the document (as opposed to stand alone sections) 4 A description of at least two measures for each social determinant goal that will be tracked over time Love to see: Transformational changes 5The desired system change. (This would be transformative!)
