1 Presentation of the Online Pre-Screening Process on EasyAppsOnline

2 Login Screen The employer will distribute a copy of the instructions to each employee. These instructions tell employees what type of information they will need and what website address to visit They will click the First Time User button to begin the log in process.

3 Employee Account Retrieval The system will retrieve their account by asking them to provide their: Last Name Year of Birth Month of Birth

4 Retrieval of Employee Credentials The system will locate the employee’s account and allow them to log in using the specified Username and Password admarx password

5 User Creates a Unique Password Employee will create a new secure password to use when future access is required.

6 User Creates Security Question EasyAppsOnline uses a two-factor authentication system therefore a Security Question and answer must be setup.

7 Employee Enrollment Main Menu  The system will brief the employee on their enrollment schedule and available benefits being offered.  To Start the Application, Employees will simply click Apply Online

8 Security Policy and Online Agreement Screen Employees will review the Security Policy and Online Agreement and then click I agree. Let’s get started.

9 Personal Information Employees will enter any information that did not default from the census uploaded by broker. They cannot proceed until all required data has been entered. This validation ensures application are 100% complete.

10 Demographics Information Employees will enter any information that did not default from the census uploaded by broker. They cannot proceed until all required data has been entered. This validation ensures application are 100% complete.

11 Job Information Employees will enter any information that did not default from the census uploaded by broker. They cannot proceed until all required data has been entered. This validation ensures application are 100% complete.

12 Employee Coverage Elections and Beneficiaries Employees will elect their coverage. Dependent coverage elections will follow on a later screen. If Life is elected, employees will enter the Primary and Contingent Beneficiary for life insurance if applicable. They can enter NONE or NO for the Contingent Beneficiary if they do not have one. Select Continue to Dependents

13 Dependents Employee adds or edits dependents and clicks SAVE for each dependent that is entered.

14 Dependent Coverage Elections Employee checks the box next to each coverage option for each dependent listed. Leaving a box blank means coverage is being waived. Employee MUST Elect or Waive all coverage offered. If employee waive MEDICAL they will not encounter medical questions and will only provide previous/ongoing health coverage and sign their application. Click Continue.

15 Previous/Current Coverage - Medical Employee completes this Prior Medical coverage using the ID card they would present to a Doctor or Hospital. Employee would then answer Yes and list and medical coverage that will CONTINUE when the new coverage begins.

16 Previous/Current Coverage – Other Lines of Coverage Employee completes the same questions for Dental, Vision and Medicare. Select Continue.

17 Medical Profile Employees enter medical history for the number of years requested by the carriers the group is submitting to, including: Hospital Visits/ ER Visits Medications Note: Carrier applications are populated based on the individual carrier requirements. Example: Humana will might ask for five (5) years, but United Health Care in some states requires ten (10) years.

18 My Family’s Medical Survey- continued Employee repeats the process of adding new conditions until all have been entered for themselves and dependents. The next year they ONLY need to update their medical data. User data is held for a period of 7-years.

19 If there are NO Medical Conditions If the employee does NOT have any medical conditions then they must click the “I verify” statement before they can proceed.

20 Additional Health Related Questions The employees will answer only applicable questions. Select Continue

21 Reason for Waiving Coverage Employee clicks the reason for waiving Select I Agree

22 Enrollment Elections Confirmation Page Employee needs to review each section to verify all data is correct. Click link to go back and edit a section. Select continue when finished.

23 Carriers Terms and Conditions Employee must click Review Application to open and review the application and terms and conditions for each carrier application attached to this account. User reviews application and terms then clicks close. The check box becomes active and they click to acknowledge the terms have been read. After all applications have been viewed the user will click the AGREE button to continue. Review Application

24 Authorization To complete the enrollment the employee must read the Authorization screen and if in agreement they hold down the left mouse button and sign their name with mouse. NOTE: The signature can also be accepted through a touch screen, stylus, or any other writing tool which connects to a PC, Mac, or mobile device. Click on “I Agree” button to continue

25 Enrollment Summary Screen Employee reviews coverage's selected and can return to the Main Menu.
