4  Must be completed and kept on file for EVERYONE involved in testing.  It communicates your role with the testing and certifies that you agree to the secure administration of the test as stated in the assessment guides.  SCHOOL CODE – 4237 DISTRICT CODE – 74010 OSA SECURITY COMPLIANCE FORM

5  All test materials that contain test questions or student responses are considered secure items. They will require special handling before, during, and after testing.  Before Testing – All materials will remain in the room 207. Pick up and sign out materials before school the morning of testing in the computer lab in room 209.  During Testing – All MEAP directions must be followed verbatim from the administration manual.  After Testing – All materials will need to be collected and returned to the computer lab in room 209. Please keep all materials in a secure location in your classroom until I come around and collect these materials or you bring them to the computer lab in room 209. ALLOWABLE USE OF DATA

6 Checklist of responsibilities:  Before testing  During testing  After testing Test Administrators must INITIAL, DATE, and RETURN to MEAP Coordinator upon completion of testing. (This is proof that secure protocol was followed.) RESPONSIBILITY FORMS

7  Make sure that there are no materials on the walls of your rooms that would provide students with clues or answers to questions.  All items such as: (displays, charts, maps, tables, bulletin board materials, etc.) that contain any information directly related to Benchmarks, Common Core, etc. need to be taken down or covered. VISUAL MATERIALS IN ROOMS

8  Test administrator Manual pgs. 33-44 & 59  Students Assessed  ALL students will be assessed in their current grade level.  The MEAP reflects the prior years learning.  Each test, must be completed on the date scheduled.  The entire test MUST be completed on the scheduled date. ADMINISTRATION MANUALS

9  IEP, 504 and ELL students MUST have accommodations listed and these MUST be routinely used as part of the students’ daily instruction to be used on the MEAP.  Accommodated students with small group, read aloud, audio, or scribe accommodations may be given the assessment at a later date, but it will be with their caseload teacher.  Accommodated students’ schedules are being developed. 10  Student Prohibited Behaviors are listed in the integrity Guide. Please review these with your 1 st hour classes prior to the testing window.  Notify school administration, or school MEAP coordinator of any student prohibited behaviors.  REMEMBER: Students showing these behaviors count as not being accessed for accountability purposes. We must have 95% of each sub group assessed. STUDENT PROHIBITED BEHAVIORS pgs. 38 & 39

11  Students will need to fill out and bubble the following:  Student Name, Teacher Name, School, District (Port Huron)  Birthdate  Class/Group Number ( See attached Teacher Code Sheet. Students should fill in the code of the teacher and hour they have for the subject of test they are taking.)  Assessment date (actual date when assessment was given)  Form 1  If the student does not have a barcode, just have them fill out a blank one and I will add a barcode sticker after testing. ANSWER DOCUMENTS pgs. 39 & 40

12  Test Administrators must be certified teachers employed by the district.  Paraprofessionals may be used as proctors, but may not administer the test directions or handle materials.  Neither can give any help on the test of any kind. WHO CAN GIVE THE TEST? pg. 41

14  Students may take bathroom breaks, but don’t advertise that fact.  Only one student may leave at a time.  Test booklet and answer sheet must be collected when that student leaves. LEAVING THE ROOM

15  Read through reminders on pages 42 & 43.  Be sure to monitor your kids as they test. Walk up and down aisles.  Collect electronic devices and make sure students turn them off.  Read directions exactly as they appear in the administration manual.  NO highlighters, pens, or markers should be used on the answer documents.  Make sure the room is quiet and free of distractions during the entire test. GENERAL REMINDERS

16  All test are untimed. If there is a student who is WAY behind the rest who are done, call me at 300-0850 and I will come get that student to finish in an alternate place.  Test answer docs and booklets must be secure at all times.  Students may take breaks between test parts (except between Writing Day 1, Parts 2 & 3). The test parts must all be completed and test materials must be collected during the break. Students many not return to any test part already completed.  When finished, students may read a book (novel) they brought with them. They may not read textbooks or work on homework in the area for which they are testing. Have a back-up plan for students that may not have one. GENERAL REMINDERS CONT.

17 Read through everything in the administration manual on pages 33- 44 & 59 to make sure all your questions are answered before administering the test. CONCLUSION
