2 Who is being tested? Elementary School Students in Grade 5 Middle School Students in Grade 8 High School Students currently enrolled in English 11 or Students that have passed English 11 but have not passed the writing SOL test. Second Chance Seniors Students that are scheduled to graduate by August 31, 2007 but have not passed the writing SOL test. These students may or may not be currently enrolled in English 11. They have the option of taking the complete test twice. 4 Make-ups Make-ups can be given anytime after the student returns to school up to the check-in. If the student missed the multiple choice section they use the same form of the test that the other students were administered. If they missed the multiple choice section of the test they must use the alternate prompt for the make-up. The alternate prompt can not be administered prior to March 8. 5 Check-in Both answer documents and secure materials will be checked in at the same time. The check-in date has been moved back one week. 6 Second Chance Senior (Term Graduate) Information Multiple Choice form- W5035;Writing Prompt-242. Bundle Second Chance Senior answer documents together. Write TERM GRADUATE on the header sheet.

7 Testing Status This must be completed if the student missed the multiple choice portion or the direct writing portion or both. Complete only one of the first five circles. Do Not grid 1 until all make-up testing is finished. Circles 7,8,10 & 11 may not be used unless directed to do so by the DDOT. 8 Testing Status Grades 5 & 8 Grade 11 No IEP/LEP exemption allowed.

9 Testing Accommodations The student must have either an IEP/504 plan or an LEP plan. Grid the accommodation if used on either or both parts of the test. 10 New Forms:Header Sheet Use my name, phone number (434-432-2761) and the appropriate date.

11 Forms Required at Check-in Include the following in order in box 1: School Transmittal Form Signed School Affidavit Signed School Security Agreements Test Booklet Assembly ID Sheets for all opened packs of Test Booklets, completed and signed. Writing Prompt Assembly ID Sheets for all opened packs of Writing Prompts, completed and signed.
