4. At TRANSITIONS, we realize that many seniors live alone or with a loved one who works during the day (thus worrying about their senior loved one while they work); are they eating? Is it too warm, too hot in the home? Did they fall? Are they hurt or experiencing a medical trauma?... 5. This program is for Seniors who live alone or with a loved one and their families. There are a myriad of day programs albeit most are ½ day and many do not provide transportation.
