PBIS Leadership Team Must have a building-wide team that oversees all development, implementation, modification, and evaluation activities in order to implement procedures and processes intended for all students, staff, and settings 10/21/08

Responsibilities of the School-wide PBIS Team Assess current behavior management practices Examine patterns of behavior Obtain staff commitment Communicate effectively with all stakeholders Develop a school-wide plan Obtain parental participation and input Meet at least monthly to oversee, monitor, and evaluate all data and planned objectives and activities developed by team Report outcomes to Coach and District coordinator 10/21/08

Positive Behavior Supports: Tier 1/Universal Action Plan Include the development, implementation, and management activities of your plan. All Critical Elements should be addressed within your plan. Critical Elemen t Action/ActivityWho is responsible? When will it be started? When will it be completed ? How will it be monitored? When will we evaluate it? #_____ Critical Elements 1.PBS Team established (membership, meeting times, roles, mission) 2.Basic PBS principles and behavior problems within school shared/reviewed with staff 3.Faculty commitment is established and maintained throughout the school year 4.Discipline data system is meaningful, data entered at least weekly, and analysis plan established 5.Discipline referral form compatible with SWIS Big 5 reports 6.Behaviors defined and categorized (major vs. minor) 7.Discipline referral process established and flow chart developed (including the development of a consequences hierarchy for classroom and office managed behaviors) 8.3-5 School-wide Expectations developed and posted 9. Behavior expectations developed for non-classroom settings 10.Lesson plans developed for teaching school-wide and non- classroom expectations 11.School-wide rewards system established (what, when, how) 12.Plans developed for training students (initial and booster) 13.Plans developed for training staff on PBS components (PBS principles, school-wide and non-classroom area expectations- initial and booster, referral process, definitions of problem behaviors, majors vs. minors, data-based decision-making) 14.Crisis procedures taught to all staff and posted throughout school 15. Plans developed for involving families and community 16. Implementation plan established (what, when, how) 17. Evaluation of PBS activities
