Don’t worry about adding pages – the first page is already done and you shouldn’t need more than one page. 3. The important box is Add Question – all sections are called questions – even introductory text boxes

Next copy and paste your introductory text and survey description into the Descriptive Text box – that is all you really need to do on this page.

We would like you to take a few minutes, as one of our valued patients, to tell us how you feel about the health services we provide to you. For each question we will give you a selection of 5 answers; just choose one and tick the box. This survey is completely confidential and anonymous.

Outer north east London Barking and Dagenham, Havering, Redbridge and Waltham Forest We do not ask for personal contact details - although we do ask that you tell us something about your gender, race, religion etc to ensure we are reaching all our communities in the area, so that we get a very clear picture, and we can talk to harder to reach groups. This survey is open from 1 February and closes on the 1 March 2012. We will use these results to inform our work here and will share the results with our Patient Participation Group - which is attached to the surgery. If you have any questions please email commmunications@onel.nhs.uk. Once you have completed the survey, click the ‘send’ button at the end. When you have entered your descriptive text, click the ‘save & add next question button’ at the bottom of the screen. 2. Enter the next question type - click the drop down arrow and choose Multiple Choice (only one answer) 3. Type your first question in the Question text box at the top 4. You then type your five multi choice answers in the Answer Options section in the Answer Choices box – each answer must be on a new line

Outer north east London Barking and Dagenham, Havering, Redbridge and Waltham Forest Once you have entered your question and answers, you should also tick the ‘Requires an answer’ to the question box – which will make the question compulsory if you want to. Click save and add next question box and enter all your questions.

Outer north east London Barking and Dagenham, Havering, Redbridge and Waltham Forest At the end of your questions it is a good idea to add a third question type – select Comments/ Essay box. Here you can ask if the patient or respondent has any other comments to make.

Outer north east London Barking and Dagenham, Havering, Redbridge and Waltham Forest Adding demographic questions Once your main survey is completed, you can add questions on gender, sexuality, race, religion and age with the same questions. Save and view your survey.

When you have entered your final demographic question – enter a final text box to remind patients to press send and thank them again. 2. Press the save button and close the questions dialogue box and this will take you to the full survey. 3. Go back and review your questions and check them for any typos.

Decide how you would like to collect responses – if you select Email then you can use the email system in surveymonkey which is really very good. 2. Enter a name for your responses collector in the name box – your name will do it

Add recipients email addresses to the list – you can copy and paste your patient email addresses in here, as long as you separate each one with a comma 2. You can then write your email by clicking the create message box. This system will also allow you to send reminders out automatically to those who have not already replied.

Outer north east London Barking and Dagenham, Havering, Redbridge and Waltham Forest Collecting and analysing responses Once you have set up your survey go back to the editor and press ‘send survey’ – bingo ! Your survey has been sent out. You can check how many people have replied each time you log in. Finally you need to close your survey and analyse your results.

Choose your format – it is best to choose Columns ‘expanded ‘ 3. Send results to this email address – add in your email address. 4. Then press the Request download button
