3 I. Program Overview California law authorizes DLC and the four OPOs to register an individual using an internet- connected device By collecting information from an individual By collecting information from a driver license or DMV-issued ID card

4 A. User Requirements Each user must: Be an active Donate Life Ambassador Pass a criminal background check Complete eRegistration training Read and electronically sign: Confidentiality Agreement Acceptable Use Policy

5 B. Approved Devices Only use OPO-issued devices Do not install or remove software/applications Do not use a personal device Not for personal communication/entertainment

Confidentiality Agreement & Acceptable Use Policy All users are required to agree and sign the agreement the first time they login to the eRegistration application It will be required every year on the anniversary of the first login The information is kept confidential and secure by Donate Life California Do not shared login information Each user has a unique username and password

7 II. Using the Device Turn on the device by holding down the power button for three seconds Enter the passcode to unlock the device provided to you On/Off button Asus Nexus 7 Volume buttons Camera lens

8 Ensure the Wi-Fi or cell signal is activated Open the Donate Life California application Enter in your assigned username and password Login Screen

9 Enter in your assigned username and password Confirm the “Network Status” is connected Confirm the Server URL is https://dlcmobile.onelegacy.org Login Screen

10 Read and sign the Confidentiality Agreement Read and sign the Acceptable Use Policy Agreements Check the box and then click “Accept”

11 Event/Campaign Name Enter in the name of the event as well as the date.

12 Home Screen Read or have the individual read the information on the screen. If the individual agrees, click “Accept.” After acknowledgment, click “Accept”

13 Home, Privacy and Exit On the navigation bar, you will find three buttons. For the DLC privacy statement, click “PRIVACY” and it will open up the web browser to donateLIFEcalifornia.org Home Privac y Exi t

14 Information Screen There are three ways to enter in information: 1.SWIPE the driver license or ID care using the external card reader 2.SCAN the back of a driver license or ID card with the device’s camera 3.ENTER the personal information into the data fields

15 SWIPE The external magnetic card reader connects to the tablet by inserting the metal piece into the headphone port. Ensure the device is properly connected by firmly pressing down so no metal is showing. On/Of f butto n Camera lens Swipe device

16 SWIPE The thicker part of the swipe device will face you The thin side will face away from you Swipe device

17 SWIPE If the card reader is NOT connected properly, the “Magnetic Swiper Status” will be set to “Disconnected.”

18 SWIPE When the card reader is connected properly, the Magnetic Swiper Status will be set to “Tap to Swipe.” Click on “SWIPE.”

19 SWIPE When swiping the driver license or DMV-issued ID card make sure the magnetic strip located on the back of the card is facing you. Be sure to run the card through the swipe device as quickly as you can.

20 SWIPE After a successful swipe, the data will populate the fields below and a “Successful swipe!” message will appear.

21 SWIPE If unsuccessful, an error message will appear. You can either repeat steps 3 to 4, or you may use the SCAN method or ENTER in the data.

22 SCAN Click the blue “Scan” button. This will activate the device’s camera.

23 SCAN Point the camera to the 2D barcode on the back of the driver license or ID card 2D barcode Magnetic strip

24 SCAN A screen will appear with lines of data. Click “Use.” Click “USE”

25 SCAN If it does not have a 2D barcode on the back of the driver license or ID card, you may use the SWIPE or ENTER in the data 2D barcode

26 ENTER IN DATA To enter in the individual’s information, click on each field and type using the on-screen keyboard.

27 ENTER IN DATA To enter in the date of birth, click “Select” and a calendar screen will appear. “Select”

28 ENTER IN DATA Begin by clicking on the year and scroll to find the appropriate year. Click on the year

29 ENTER IN DATA Then select the month and date. Click “OK” when done. Scroll to find the month and select the date

30 Reviewing the Information Once the information is entered, review the information to make sure all required fields are entered. Then have the individual confirm the information.

31 Reviewing the Information If any information is incorrect, go back and manually enter in the correct information. Tap the field to edit

32 Reviewing the Information When all the information is correct, have the individual read the consent statement and click “Accept.”

33 Signature Screen Ask the individual to provide a signature using their finger.

34 Signature Screen If the individual is under the age of 18, it will ask if a parent is available to sign.

35 Signature Screen If yes, it will ask for the first and last name of the parent.

36 Signature Screen It will then ask the parent for a signature.

37 Signature Screen If a parent is not available, click “no.”

38 Receipt Screen The individual will be asked if they would like an email receipt.

39 Receipt Screen If yes, select “yes” and enter in the email address using the screen’s keyboard.

40 Receipt Screen If no, select “no.”

41 Confirmation Screen After a successful registration, a “Registration successful!” message will appear.

42 Confirmation Screen To continue on to register the next individual, click “Home.”

43 Confirmation Screen If you are done using the application, be sure to Exit by click the icon at the top right of the screen. Exi t

44 Turning Off the Device Most devices have a power button located either on the top or the side of the screen. Press and hold the button down for three seconds to turn the display black/off. On/Off button Asus Nexus 7

45 III. Privacy Policy Donate Life California has adopted and adheres to stringent security standards designed to protect your personal information from unauthorized access or disclosure. Among the safeguards that we have developed for our site are administrative, physical and technical barriers that together protect the information stored at this site. No information is ever stored in the application, to the device or device memory.
