4. Funeral Home  Must be located at a specific address  1,250 sq feet of space  Preparation room or refrigeration or contract for these services  FDIC  Annually inspected  Biennially renewal of license

5. Funeral Home  Embalmers must have license  Identify all principals on application  Must display name of FDIC at public entrance  All licenses must be posted in public area  No co-location of funeral homes and direct disposer after 10/1/1993

6. Funeral Home  Direct Disposer can not practice direct disposition in a funeral home!

7. Visitation Chapels  Each funeral home may operate a visitation chapel at a separate location within the county the funeral home is located in.  500-700 sq feet in size  Can only be used for visitation  Must have licensed FD present during visitations

8. Funeral Director in Charge  Responsibility of the FDIC  Make sure they are operating legally  Available within 2 hours by telephone  Make sure there is a funeral director available  Oversee all pre-need contracts if they have a Pre-need License (COA)  Notify to Brd. within 20 days of new FDIC

9. Funeral Director  Able to perform all duties within the funeral home except embalming. Can only practice at a license establishment FD and Embalmer license

10. Funeral Director requirements  18 yrs  Convicted of a crime  AS degree or higher http://www.abfse.org/ http://www.abfse.org/  Pass the NBE http://www.cfseb.org/ http://www.cfseb.org/  Year of Internship  Pass 75% Rules and Regulations test  Communicable disease class

11. Embalmer License  License to embalm  Same procedures as FD but:  Complete one year of school in embalming sciences  Pass the Science section on NBE

12. License by Endorsement  FD & Emb who holds a valid license from another state.  Similar qualifications  Passing the rules and regulations exam with a 75%  Prior to 1979 exemption from Associated degree

13. Temporary License  Valid license from another state can have up to 6 months to work under the direction of a license funeral director and embalmer

14. Provisional License  Individuals who have met all requirements to sit for examination for licensure may apply for a provisional license. Must practice under direct supervision of a license individual.

15. Practice of Funeral Directing  Selling or offering to sell funeral services on a at-need basis.  During the arrangement conference  Setting the time of service  Establishing the type of service  Acquiring the services of clergy  Obtaining vital information for the DC or burial transit permit

16. Practice of Funeral Directing  Making or negotiating financial arrangements for a funeral service  Directing or supervising funeral service in a funeral home, cemetery or elsewhere  Directing a visitation, but does not need to be physically present throughout the service, but does need to be in touch

17. Funeral Director and Embalmer re-licensure  Continuing Education requirements  12 CEU’s  1 CEU for HIV/Communicable Diseases

18. Funeral Director and Embalmer Intern  Internship – 12 months period of time working at an approved training agency.  Funeral Home must provide 40 cases per intern.  Internship is served after completing their education requirements

19. Embalmer Apprentice  Any person desiring to become a funeral director and embalmer may apply for embalmer apprenticeship.  18 yrs old  High school or GED  Serve up to 3 years  Work under direct supervision

20. Direct Disposer Establishment  Can only arrange for immediate cremation. No services  Funeral Directors working at DD establishments can only do direct cremations.  No viewings, memorial services or any type of gatherings.  No ID viewings!

21. DD duties  Removal from place of death  Vital statistics information  Necessary permits for cremation  Arrange for obituaries as long as name of DD establishment is not listed with memorial service  Refrigerate remains  Contract with removal services and crematories

22. Direct Disposer Establishments  Director Disposer can only practice here  625 square feet at physical address  Name of DDIC at public entrance  Refrigeration or contracted for services within 75 miles  Biennially renewal and annually inspected  May not operate at same address as other establishments unless were collocated before 7/1/2000

23. Direct Disposer  18 yrs old  High school or GED  Conviction of a crime  Passing grade in college credit course in Florida funeral law  Pass examination by Brd. on rules and regulations  2 ceu’s course on communicable diseases

24. Direct Disposer  Continuing education requirements for relicensure  2 CEU’s on HIV/communicable diseases  3 CEU’s in Health, Safety or Laws and Rules

25. Centralized Embalming Establishment  Provide embalming services for funeral establishments only  Meet all requirements of a funeral home preparation room  Embalmer in Charge  Report cases handle  Annual inspection biennially renewal

26. Centralized Embalming Establishment  Must have identification process for all bodies.  Can not work for the public

27. Criteria for licensure  Refrigeration equipment large enough to store all bodies found in the facility  Proper exhaust system  Non-porous surfaces  Floor drain or equivalent  Embalming instruments  Embalming chemicals

28. Criteria for licensure  Disinfecting chemicals  Sealed containers for transportation of bodies  Bio Medical waste procedures  Asked for receipts

29. Cinerator Facilities  Facilities where dead human bodies are reduced to a residue, including bone fragments by direct flame, also know as cremation or by intense heat, also known as calcination.

30. Cinerator  Biennially renewal with annual inspections  Goes null and void if not renewed  Must be under direction of LFD or DD  FD if located at funeral home  May only cremate dead human bodies after 48 hrs  Monthly reports of persons cremated

31. Cinerator  All bodies must be cremated in a covered container or casket. The container must be rigid enough to provide easy handling and leak proof.  Single cremation unless authorized by next of kin

32. Removal Services  Make removals for funeral homes and direct disposer establishments  Inspection should:  Clean and sanitary  PPE’s  Cots and covers  Body bags  First Aid kit  Fire extinguisher

33. Removal Services  Must have identification process

34. Refrigeration Services  Not connected with a funeral home, crematory or direct disposer. Provide refrigeration services to funeral homes and direct disposer facilities.  Bodies must be kept below 40 degrees

35. Broker of Burial Rights  No person may receive compensation to act as a third party to the sale of transfer of three or more burial rights in a 12 month period unless registered with the Board (Brd).  Does not apply to anyone register with Brd.  Records must be maintained  Biennial renewal

36. Preneed License  Funeral home, cemetery, direct disposer establishment or monument dealer may be licensed to sell preneed if approved by the Brd.  This contract can be for services or merchandise

37. Pre-need agents  Must be under contract or employed by the preneed license they represent  Can be under more than one preneed license  18 yrs old  Biennially renewal

38. Monument Establishments  Sell markers, monuments and memorials to public.  Not associated with licenses  Must have an office, reception area and display  Open to public  Must meet local zoning laws

39. Monument Builders  Can apply for preneed license  3 years experience  Net worth of $10,000  Alternate methods to meet net worth
