Casey An adequate ASE has been developed in accordance with the ASO and is supported by the SAD. Casey - general Hettel – operational requirements adequately defined Job – radiation safety requirements adequately defined An appropriate commissioning plan has been developed. Casey - general Hettel – operational plans consistent with SAD and ASE requirements Job – radiation assessments to confirm adequacy of shields and beam dumps for normal and abnormal conditions An appropriate USI process has been developed. Casey Procedures necessary for the safe operation of the activity have been developed, reviewed, and approved, and an appropriate process for the development, review and approval of new and revised procedures is in place. Hettel – are adequate procedures to conduct undulator complex commissioning and fault studies in place Job – are adequate procedures in place for fault studies; radiation monitoring, beam containment systems testing, certification and configuration control Markovich, Forrestal – are adequate procedures in place for search and secure; interlock testing, certification and configuration control Evans – ODH & electrical safety Floyd – ISM processes, including work planning, review and approval of facility mods; management of safety related changes Buckley – conduct of operations program; process for development, review and approval of new and revised procedures Procedures to deal with abnormal and emergency situations have been prepared and are approved for use. Evans – ODH, fire, radiation, electrical, medical Hettel – Main control room response Now assigning SLAC contacts.

Appendix B: The review process used during LCLS Undulator Complex engineering. Safety and oversight reviews are discussed, and field inspections by Safety Officers and committees are documented. Appendix C: Procedures and training required for operation and maintenance of LCLS Undulator Complex commissioning. Appendix D: Plans for beamline commissioning. Beamline component checkout, system training for Operators and the schedule for operations, including radiation measurements. Appendix E: Draft Beam Authorization Sheet for the LCLS Undulator Complex.
