3 Target Population Prioritization through Coordinated Access and Assessment (CAA) Individuals over 18 years of age Chronic and Episodic Homelessness High Acuity SPDAT Score No Exclusionary Criteria Existence of complex co-occurring conditions

20 Proposal Preparation A checklist of required proposal documents to be submitted is included in Schedule A of each RFP Ensure all Mandatory Requirements are met Proposal packages should be submitted 3 hole punched for consistency, containing all information and responses to all sections. Do not use the agency name anywhere other than the front page of the proposal submission.

21 Proposal Preparation Submit eight (8) hard copies and one (1) electronic copy of your proposal. Schedule B to be in excel format (not pdf) 1 USB key with all documents in electronic format Proposals must be submitted in a sealed package, the outside of the sealed package should be prominently marked with the RFP title and reference number, the full legal name and return address of the Proponent and the Proposal Submission Deadline date and time. 21
