1. IMPORTANT NOTICE The QMA now offers Group student insurance plan without proof of good health.

2. The following coverage for medical students are now offered without medical proof : $ 50 000 - life insurance $ 50 000 - accidental death and dismemberment insurance $ 1000 - disability insurance with 90 days waiting period (1 st et 2 nd level) $ 1500 - disability insurance with 90 days waiting period ( 3 rd and 4 th level) Premium has been waived :  Reduce disability rider  Cost of living adjustment rider  Future insurance option rider

3. The annual premium : 1 st and 2 nd level : $ 60 3 rd and 4 th level : $ 90

4. Benefits will not be payable for a period of Total / Partial disability resulting directly or indirectly from a condition for which the student should have sought treatment or advise during the 12 months prior to the effective date of coverage. However, this limitation will not apply to any disability that begins after the coverage has been in effect for a period of 12 months from the effective date of coverage. Pre-existing conditions

5. Coverage can be transferred to the Residency Plan No new proof or medical declaration to provide when transferring to our plans for residents and physicians.
