How to Request a Course

When to start??? Employees who’ve not worked for UNM in the last year may need to reapply or provide updated paperwork. If the instructor needs to be hired or re-hired by UNM, a 12 week lead time is required for course to allow for time to hire instructor. To become a current instructor at UNM, apply at https://unmjobs.unm.eduhttps://unmjobs.unm.edu. Otherwise 8 weeks prior to course

What course are you requesting? What is the type? How many are coming for sure? What is the maximum amount of students allowed? Is it open to the public or is it for a select group only? Instructor Information Does the course have an instructor yet? Will there be an assistant? Information you’ll need for the Instructor and Assistant Name Instructor Coordinator # Demographic Info Planned # of teaching hours If there is no assistant instructor “N/A” will suffice Don’t Forget Can the instructors perform the requirements for this course? How are they being paid?

Course Location Information Where will the course be located? Physical Address Please don’t forget to put room numbers if applicable Where will the clinical shifts be held?? List preferred and/or confirmed sites here

Course Information What is the Course Start Date? What is the Last Date of the Instruction? What is the anticipated date that students will complete their clinicals? What are the scheduled course dates and times? Check the day and enter the start and end time for that day. Don’t forget to enter AM/PM or use Military Time

Paperwork and Materials Please indicate whether you will be picking up the paperwork and course materials or provide us with the address where you would us to send them to. * Please Note: We anticipate transitioning away from Bubble Sheets in the future. Course Packets include Student Registration Forms* Release Forms Grading Sheets* Evaluation Sheets* Clinical Requirements Clinical Timeline Fingerprint Cards Instructor CD (if applicable) Textbooks (when applicable) Are for initial licensure courses only Contact EMS if you are unsure of where students can pick up their textbooks

Supplies Requested Please indicate whether or not you will need supplies for this course. If you need supplies REMEMBER to enter a deliver by and pick up by date (if applicable). Please sign in the box at the bottom of the page to indicate that you have reviewed and approved the supply list. If you need limited supplies please adjust the supply form accordingly. The Trailer Inventory (supply list) is page 3 of the Course Request Form.

Important Note Please ensure that we have enough time to order the necessary supplies. If you are submitting your course request late, or forgot to include the supply list when you initially submitted your request, you may not have all your supplies in time

Agreements Please review and check all that apply. If a PO will be the form of payment we MUST have a billing contact name and phone number.

Comments and Signature If there is any information that you feel is necessary please feel free to write comments. This makes the process easier on everyone. Please sign and date prior to submitting. We also accept E-Signatures.
