Summer Retest Alerts for Test Coordinators Assign TA numbers June 2–July 1. The Test Administrator System is refreshed for summer testing. Verify that all used documents have a TA number coded in field H2. Use security checklists to track materials. Electronic copies are available on LEAPweb. Be sure to indicate on the checklists which unused answer documents received Test Not Taken labels. When packaging materials for return, stack secure materials according to security numbers. This will aid in verifying materials against security checklists. Pickups 1 and 2: Used answer documents for more than one grade or content area may be boxed together. Pickup 3: Test booklets and answer documents must be boxed separately, but materials for more than one grade and content area may be boxed together. Return large-print and braille test booklets in their original shipping boxes.
7
Use the provided plastic bags for the return of approved home study program and LEAP nonpublic school students’ answer documents to the District Test Coordinator. Nonpreidentified labels from testing sites must not be affixed to answer documents for these students; the District Test Coordinator will affix nonpreidentified labels with site code 998 or 999 after testing. Return district office documents (security checklist copies, testing irregularities, the Oath of Security and Confidentiality Statement) in the provided envelope. The District Test Coordinator should return the envelope for subsequent pickups. Summer Retest Alerts for Test Coordinators

Sample GEE Student/Label Roster These columns indicate the content areas in which students are identified as needing to retest. For students who do not receive preidentified labels, record the numbers from the nonpreidentified bar-code labels affixed to their answer documents.
17
Sample LEAP Student/Label Roster LEAP students will receive a preidentified label for English Language Arts and/or Mathematics. For students who do not receive preidentified labels, record the numbers from the nonpreidentified bar-code labels affixed to their answer documents.

Used and Unused Answer Documents USED (PICKUPS 1 AND 2) UNUSED (PICKUP 3) All used answer documents are returned in pickups 1 and 2. Used answer document: an answer document on which a student has marked answers to one or more items and on which a preidentified bar-code label has been affixed to page 1 a hand-coded answer document on which a public school or post-high school student marked answers to one or more items and on which a nonpreidentified bar- code label has been affixed to page 1 an answer document on which a student enrolled in an approved home study program has marked answers an answer document on which a student enrolled in a LEAP nonpublic school has marked answers All unused answer documents are returned in pickup 3. Unused Answer Document: An unused answer document is one on which no test items were attempted. For the summer retest, an answer document containing no student responses is considered unused regardless of whether a preidentified or nonpreidentified bar-code label has been affixed or whether student information has been coded on page 1. Test Not Taken labels must be affixed to unused answer documents that have a bar-code label and/or hand-coded student information. All unused answer documents—those that require Test Not Taken labels and those that do not—must be returned. SUMMER 2008 RETEST ONLY

Preidentified Bar-code Labels For the summer test only, preidentified labels should be affixed to answer documents only for students who are expected to test. Unused bar-code labels must be returned with pickup 3 materials. The District Test Coordinator will distribute all preidentified labels to summer testing sites before testing sessions. Students who have been identified as needing to retest will receive a preidentified label. This includes students who took the GEE February Seniors Only Retest. If a student took no part of a test and did not attend makeup testing, return the student’s unused answer document with all other unused answer documents for the testing site. The School Test Coordinator must affix Test Not Taken Labels to unused answer documents that have bar-code labels and/or hand-coded student information. NonPreidentified Bar-code Labels If a student does not receive preidentified labels and must test, the test administrator must affix a nonpreidentified label to each of the student’s answer documents. The test administrator must hand code student identification information on each answer document according to directions in the Test Administration Manual. The fields must be coded exactly the same on all answer documents for a student. If a student receives a single preidentified label but needs to test using two or three answer documents, the answer document with the nonpreidentified label must be hand coded to match the information on the preidentified label exactly. ^ ALIGN TOP OF LABEL ^ IIIIIIIIIIIIIIIIIIIIIIIIIIIIII L51080 9399999909 000 Bayou High School 001 Pelican Parish SUMMER 2008 BAR-CODE LABEL

Approved Home Study Program and LEAP Nonpublic School Student Labels The District Test Coordinator will distribute to appropriate summer testing sites preidentified labels for approved home study program and LEAP nonpublic school students. If approved home study program or LEAP nonpublic school students do not receive preidentified labels, the test administrator must hand code the answer documents but must not affix labels from the summer testing site to these answer documents. The District Test Coordinator will affix nonpreidentified labels to these answer documents after testing. School Test Coordinators will place all answer documents for approved home study program and LEAP nonpublic school students in the provided plastic bags and give them directly to the District Test Coordinator. The District Test Coordinator must remove these answer documents from the bag, affix nonpreidentified labels with site code 998 or 999 to those without preidentified labels, and return these answer documents to DRC in pickup 2. See page 14 in the Test Coordinators Manual for returning test materials for approved home study program and LEAP nonpublic school students in pickup 2.

Test Not Taken Labels (affixed by School Test Coordinator) After all makeup testing is complete, the School Test Coordinator must affix yellow Test Not Taken labels to unused answer documents that have a bar-code label and/or hand-coded student information. Each unused answer document with a bar-code label or hand-coded student information must be inspected for student responses before affixing the labels. Test Not Taken labels should be applied over preidentified or nonpreidentified bar-code labels. Unused answer documents without bar-code labels or hand-coded student information do not need a label. The School Test Coordinator must indicate on the Testing Site Security Checklist which unused answer documents received Test Not Taken labels. Unused answer documents without bar-code labels or hand coded student information do not need a label. Unscorable Document Labels (affixed by the District Test Coordinator) UNSCORABLE DOCUMENT LABEL (BLUE) INSTRUCTION SHEET (PROVIDED TO DTC WITH SHEET OF UNSCORABLE DOCUMENT LABELS) Unscorable document labels must be used when an answer document is soiled or damaged or when a student took a test on an answer document with a preidentified label for another student. The damaged document must not be thrown away, and the test administrator must contact the School Test Coordinator (STC) immediately if an answer document should be designated unscorable. The STC then must contact the District Test Coordinator (DTC) immediately. The DTC will provide instructions for transferring student responses to a scorable answer document. The new scorable answer document must be returned by the school with all other scorable answer documents. The DTC will affix a blue Unscorable Document label to the damaged answer document and return it with the unused district overage answer documents in pickup 3. Documentation regarding the incident must be kept on file at the district office for one year. ^ ALIGN TOP OF LABEL ^ THE DISTRICT TEST COORDINATOR MUST FOLLOW THE SPECIAL INSTRUCTIONS PROVIDED WITH THIS LABEL.
