We are pleased to announce
Your new Health and Dental plans will be transitioned to Manulife effective May 1, 2017. Your benefits will largely remain the same and you will continue to enjoy the same coverage as was in place prior to the transition with the exception of some enhancements (e.g. Psychotherapist). All unfiled health and dental claims submitted after the transition date should go to Manulife, regardless of when the expense was incurred. 3 3

Go to the Teledyne Portal at manulife.ca/TeledyneCanada First time users or new employees click REGISTER or click SIGN IN if you already registered. 6

Activating Your Account Online
Fill in all of the information on the registration page. Refer to your benefit card for plan contract and certificate numbers. 7

You will receive your activation key shortly after the effective date. Enter your activation key exactly as indicated in the letter. This key is only necessary the first time you login. For future logins, use the password you assigned during the registration process. If you have not received an activation key by the second week of May, contact for assistance

Fully electronic solution
Like online claim submission, Provider eClaims is fully electronic. You must be registered and activated on the plan member secure site. You must have direct deposit and electronic claim statement. For provider payment, provider must have direct deposit No paper cheques No paper claim statements 14

Provider eClaims – What is it?
Submit Claim is submitted directly from the provider’s office. e.g. Physiotherapist, Chiropractor, Massage Therapist. Adjudicate Adjudication while you are in the provider’s office. In most situations, you and your provider will know right away how much is covered by the plan. Assign Payment Direct payment to the provider (assignment). You are out of pocket only the amount not covered by the plan. Or, save and pend If we can’t adjudicate the claim right away, nothing more for provider or you to do. Provider is notified, claim is saved and adjudicated later. 15

Coordinating your benefits may allow you to recover up to 100% of your claim, ensure Manulife is aware of any other coverage in place for your spouse/children. Talk to your doctor or pharmacist about generic drug options. Ask for a 3-month supply of maintenance medication. Get an estimate for dental expenses. Due to confidentiality, banking information will not be transferred from Sun Life to Manulife. Please visit the plan member secure site to update this information. Please refer to the FAQ on your portal for more information. 19
