Classic sniffing position or optimal head position for direct laryngoscopy and the 3-axes alignment theory. Classic sniffing position or optimal head position for direct laryngoscopy and the 3-axes alignment theory. The oral axis is defined as a line drawn horizontally across the top of the tongue. The pharyngeal axis is a line tangential from the uvula to the posterior surface of the epiglottis. A: The laryngeal axis is a line drawn at a right angle to the vocal cords. B: Head extension at the atlanto-occipital joint more closely aligns the oral axis with these 2 axes. From Reference 25, with permission.
