2 Continuous Process Improvement (CPI)
The purpose of the CPI program is to monitor and track the submission of required data by network transplant centers. CIBMTR will eventually bring all centers into a CPI program but is rolling out the program regionally. Australia and New Zealand are first Brazilian centers will be added later in 2019 Applicable centers will begin receiving CPI program updates in 2019 outlining progress toward meeting CPI requirements.

Requirements of CPI: Signed Data Transmission Agreement (DTA) or updated Master Healthcare Data and Sample Submission Agreement (MHA) Consecutive list of all cell transplants beginning in 2016 Include allogeneic related, allogeneic unrelated, autologous Do NOT include cellular therapies Minimum of 90% of forms completed error free for transplants performed from 2016 to present

4 Consecutive Reporting of HCTs to CIBMTR
First cycle starting in 2019 for applicable centers Submit list of all HCTs that occurred January 1, 2016 – December 31, 2018 Include patients with one to multiple transplants Do NOT include cellular therapies If you perform, but do not report autologous HCTs to CIBMTR, do NOT include them in your list If your center is not part of the pilot, please begin to keep track of your center’s infusions so you are ready when your center goes live.

5 Consecutive Reporting of HCTs to CIBMTR
Consecutive list must be in Excel format and contain these data elements CRID – (if applicable); in a NUMBER format DOB – formatted as a DATE FIELD (yyyy-mm-dd) Sex – M, F (Male, Female) Date of HCT - formatted as a DATE FIELD (yyyy-mm-dd) HCT Type – Allo_R, Allo_U, Auto (in this format) Do NOT include recipient names or any other variables Make sure all applicable infusions have CRIDs to save time!

7 Forms Due Requirements
Applicable centers will receive forms due lists before the end of year. Look for details and trainings to come All centers can expect an updated weekly forms due list in the future
