INCIDENT One of the employees working as a helper, was trying to connect two luggage carts (trolley) together with the pad eye connector. His right hand middle finger was trapped between the trolleys & led to injury of his finger. ROOT CAUSES: Improper handling of the trolleys by IP led to finger being trapped between the two trolleys IP failed to identify the hazard involved in connecting the trolleys Sana’a Desert Trading Company (SDTC) risk assessment does not include the risk & hazards involved in connecting the trolleys. The pinch point on the trolleys was not indicated to warn the user.
