6 Methodology Order bus - 1 month delivery time
Anticipated delivery: February 2014 Survey residents for activity requests Plan activities and distribute monthly calendar Sign up for off campus activities 2 weeks before Begin using bus for medical appointments immediately Begin scheduled off campus social events in March 2014 The bus would be used on an average of 4 to 5 times daily. Between a new bus and our current van, our drivers would be able to reach up to 20 destinations per day. This bus would be able to transport up to 14 passengers at a time, and has the capacity of carrying four wheelchairs. We would provide transportation to out-patient rehabilitation therapy patients, making a bus with enough space for wheelchairs is a necessity. When space allows, we would also provide bus service to our 3,000 community members of the Stonehill Wellness Center, especially the more than 1,500 members over age 65. Finally, we would also be able to offer the service to the seniors in the low-cost apartments at Windsor Park and Alverno, so they too would have access to the community. We would also be more effective in transporting residents to their doctor appointments. The social worker and the Director of Transportation work together to create timely and effective pick-up and drop-off routes around the city of Dubuque, so residents would not have to wait more than half an hour for the bus. We can accomplish this goal by maximizing our two modes of transportation. The van we already own can be driven by anyone on Stonehill’s insurance such as social workers and patient advocates. The Transportation Director can train members of the current staff as needed to attain certified driver’s licensure (CDL), which is required to drive the bus, which will help meet the increased transportation demand. We can ensure the new bus will have an immediate impact on the Stonehill community, as we already have the budget and staff support to make it successful.

7 Outcomes A minimum of 10 social events off campus each month
80% of residents leaving campus for a social event once a month Cutting wait times at medical appointments in half EVALUATION Changes will be tracked by the Activities Director and informally evaluated by discussions with residents Report changes in the monthly newsletters

8 Challenges of Writing Not being at Stonehill with the residents and interacting with them every day Not a typical grant submission Application form and description of organization Professional and succinct phrasing

9 Thank you!
