2. Select The Patient From The Pending List
Select The Patient From The Pending List. You will notice that the patient shows up two times. You will need to choose either the drug or the alcohol first by clicking on the donor name.

3. These patients will have both a BAT and a UDS, so be sure to complete both services in FormFox.

4. Verify the donor information. Click Capture Donor Signature.

5. Have the donor sign on the screen of the I-Pad.

6. Select the test device from the drop down menu
Select the test device from the drop down menu. Record the reading date/time, test # (this number will be shown on the test device you are using), and the screening level. Then capture the technician signature.

7. Have the collector sign.

8. Click the print ATF button.

9. Click on the print button to print.

10. To end the test, click finish.

11. Once you click finish, you will be sent back to the pending list where you will be able to select the second service to be performed for the same donor.

12. If a confirmation test is required, have the donor wait 15 minutes
If a confirmation test is required, have the donor wait 15 minutes. Then choose the test device, provide the reading date/time, the test #, and the confirmation level. Then click on submit confirmation.

13. Click on the capture technician signature button.

14. Have the collector sign.

15. Once the technician has signed, click on the print ATF button.

16. Click on the Print button

17. Click on the finish button to end the test.
