2 What’s happening? We all want Greater Manchester to be a better place to live with healthier, wealthier and happier people. We know that the things which will help people be healthier are jobs, good housing and connections to families and community and the right public services. But the opportunity is also huge and this is why we’ve done a deal with the government to take charge of the £6 billion spent on health and social services. The challenge is big; The money available for our public services is reducing every year

3 Our goal is to see the greatest and fastest improvement to the health, wealth and wellbeing of the 2.8 million people in the towns and cities of Greater Manchester.

4 Considerable more power over Greater Manchester combined health and social care budgets. Some additional support to create a transformational programme of change. Greater Manchester will remain within the NHS and social care systems and continue to uphold standards in national guidance and statutory duties in NHS Constitution. Decisions will be made at the most appropriate level to the benefit of people in GM – sometimes locally and sometimes at a GM level Organisations will work together to take decisions based on prioritising their people and their place WHAT DOES IT MEAN

7 6 The Memorandum of Understanding (MoU) states that partners will… 1.Improve the health and wellbeing of all Greater Manchester people. 2.Close the health inequalities gap faster within GM, and between GM and the rest of the UK. 3.Integrate physical health, mental health and social care services across GM. 4.Build on the Healthier Together programme.

8 7 5.Move care closer to home. 6. Do more on wellbeing, prevention and public health. 7. Contribute to and connect people to economic growth. 8. Create a partnership between the NHS, social care, universities and science and knowledge industries for the benefit of the population.

10 9 9 2012 Winter 2015 Spring 2016 Summer 2016 Autumn 2016 Winter 2016 Spring 2017 Spring 2015 Summer 2015 Autumn 2015 APRIL 2015: Process for establishment of Shadow Governance Arrangements Agreed and initiated MAY-DECEMBER: Announcement of Early implementation Priorities AUGUST: Production of an Outline Plan to support the CSR process which will Include a specific investment fund proposal to further support primary and community care and will be the first stage of the development of the full Strategic Plan. OCTOBER: Governance structures fully established and operating in shadow form. DECEMBER: Production of the final agreed GM Strategic Sustainability Plan and individual Locality Plans ready for the start of the 2016/17 financial year. DECEMBER: In preparation for devolution, GM and NHSE will have approved the details on the funds to be devolved and supporting governance, and local authorities and CCGs will have formally agreed the integrated health and social care arrangements. APRIL 2016: Full devolution of agreed budgets, with the preferred governance arrangements and underpinning GM and locality S75 agreements in place. So far...

GM Transformation Proposals and Early Implementors The Strategic Plan sets out the vision for the delivery of services within GM and what a sustainable approach would look like. The Strategic Plan provides a framework to ensure the overall level of ambition is achieved and for the development of Locality Plans and Sector Plans Each Locality (Borough) in GM has produced their own Locality Plan which explains the local priorities. The Bolton plan has been created cooperatively with LA, CCG, provider and Healthwatch input. The Strategic Plan identifies new models of care/strategies and where transformation is needed and identifies some areas where a GM wide approach will be taken forward. 4. Financial Plan & Enablers A GM Model is being developed which will be capable of predicting the impact of new models of care and of locality and sector plans. The Plan….
