4 Relationship Based Care
is the nursing delivery model of care at MGMC. Relationship based care is a nursing care delivery system that gives us a structure for organizing and delivering care with Registered Nurse responsibility, authority and accountability for outcome as well as emphasis on intentional partnerships with patients and families. The Patient is at the center of our care. The Health Care team is responsible for knowing the patient’s history or patient’s story. The Patient is involved in goal making and their individual plan of care. Use the White board to communicate the plan of care with our patients. Our computerized medical record highlights our patient's preferences and individualization needed to give excellent care.
