2 Why an application? It gives personal and job history
It allows your employer to have easy access to your personal information in one place It makes it easy to verify your information You have signed it, stating it is true to the best of your knowledge

3 Sections of the Application
Typically the application will be broken down into sections Each section is going to ask for information in one area of your life The following slides will let you know what to expect in each section

4 Personal Information Name Address City, State, Zip Code Phone Number
Eligibility to Work in US Felony convictions If under age, working paper certificate

5 Position Applied for Information
Title of the job you are applying for Hours/days available to work When you can start work

6 Employment Information
Names, addresses, phone numbers of previous employers Supervisor's name Dates of employment Salary Reason for Leaving

7 Education Schools/Colleges Attended Major Degree/Diploma
Graduation Date(s)

8 References & Resume You should include at least 3 references
Typically you don’t want more than one personal reference, unless this is your first job Always ask a reference before giving their name to someone NEVER list a family member!!!!! Attach your resume if you have one

9 Tips Complete all requested information. Don't leave anything blank. If you don't know the details, bring the application home and return it when it's completed. Write clearly and neatly, using black or blue ink. Check for spelling and grammatical errors. Proof read your application before you turn it in.

10 More Tips List your most recent job first when completing employment information. List your most recent education first. Include vocational schools and training programs as well as college and high school. References don't necessarily have to be professional. If you have volunteered you can use members of the organizations that you have helped or if you are a student use your teachers. In all cases, ask for permission prior to using the person for a reference.

11 Most Important Don't forget to sign your application!
