Thoracic aortic aneurysm status post stent-graft repair. (A) Scout image from CT scan demonstrates a bulging aortic knob due to aneurysm involving the aortic arch (arrowhead). The stent-graft is clearly seen (white arrow). (B) Coronal reformat from CT angiogram demonstrates the aneurysm (arrowheads) with a graft and surrounding mural thrombus. Contrast material within the aneurysm sac but outside of the graft (white arrow) indicates endoleak (persistent leakage of blood into the aneurysm sac). There are multiple types of endoleak, and a full discussion is beyond the scope of this text. This was felt to represent a Type II endoleak where the aneurysm sac is fed by collateral vessels, in this case arising from the left subclavian and common carotid arteries (not depicted here). (C) Sagittal reformat also demonstrates the aneurysm and stent-graft with mural thrombus in the aneurysm sac (T). A small amount of extraluminal contrast along the underside of the stent-graft indicates endoleak.
