A 55-year-old mentally challenged man with chronic sinus congestion. (A) Coronal T1-weighted image demonstrates diffuse abnormal soft tissue filling the nasal cavity and paranasal sinuses (F, frontal; E, ethmoid; M, maxillary). The material within the paranasal sinuses is of a mixed signal intensity due to variable protein content. Note the remodeling of the orbital roofs and medial orbital walls (black arrowheads) due to mucocele formation and sinus expansion. (B) On a coronal fast spin-echo T2-weighted image with fat saturation, the heterogeneous signal intensity of the thickened mucosa and proteinaceous secretions can be well appreciated. (C) Postgadolinium, an axial T1-weighted image with fat saturation demonstrates enhancement around the periphery of the markedly expanded frontal sinuses (F), but no enhancement of the centrally located mucoid material. These findings are typical of severe sinonasal polyposis and mucocele formation. Source: Chapter 3.
