TIMES OF DIVE SESSIONS Each dive session at the aquarium is limited to 6 DIVERS ONLY - if all places are full on the date and time chosen when your booking form is received we will offer you an alternative date and/or time You MUST arrive 15 minutes before the start of your scheduled dive session - Late arrivals will not be allowed to take part. SPONSORSHIP All of the minimum sponsorship, after the cost of your dive, goes directly to Muscular Dystrophy UK. Anything over the minimum amount will also go to the charity. YOUR COMPLETE £250 MINIMUM SPONSORSHIP IS PAYABLE ON THE DAY (at least £125 must be paid by current dated cheque or cash on the day or through an onlne fundraising page, with the balance of £125 ALSO PAID ON THE DAY OF YOUR DIVE by current or post-dated cheques - you may post date the cheques up to 1 month after the date of your dive to allow you time to collect in any sponsorship before your cheques are presented to the bank for payment). SPECTATORS Each diver has 2 spectators included free of charge - additional spectators must be paid for as indicated on the booking form. YOU MUST ALSO COMPLETE AND RETURN THE PADI MEDICAL QUESTIONNAIRE Please answer each question YES or NO on the line to the left of each question - you do not need to provide details at this stage. BUT if you answer YES to any of the questions, your GP will need to sign an additional form which states that you may take part in Scuba Diving. This will be provided if needed. However, even if your GP can find no medical condition which would prevent you from diving and signs you off, the Blue Planet instructors will make the final decision as to whether you can take part, if you have answered YES to any question. On the day of your dive, if you attend but are unable to complete your ‘Shark Encounter’ session for any reason, your booking fee will be non-refundable. Every effort will be made to accommodate you on another date, but cannot be guaranteed. Your sponsorship will be transferable to a new date if one is available, but a new booking fee of £30 will be required. If you are unsure of any of these details and have any questions, please call us on An experience you will never forget Muscular Dystrophy UK, in association with Blue Planet, Britain’s biggest aquarium, are pleased to offer you the unique opportunity to discover the basics of Scuba diving (with Sharks for company!) and raise funds for the charity at the same time. Blue Planet is one of the only aquariums in the world to have attained PADI resort status, which means their staff are fully qualified to make your dreams come true. A shark encounter really is the ultimate challenge as you test your nerve eyeballing the creatures of the deep. Following a thorough training and safety briefing with a fully qualified instructor, you will be accompanied for a ‘Shark Encounter’ session into one of Europe’s largest aquariums. Here you will meet shoals of jacks, grunts and snappers along with Stingrays, Nurse sharks, Sand Tiger Sharks and Lemon Sharks. You may even be able to gather shark teeth from the coral sand as the previous owners cruise past! On a Shark Encounter diving session, a group of six divers will have access to the aquarium from 2.15pm or 4.15pm depending on your session time Each diver spends about 30 minutes in the water. A guide itinerary is on the reverse of this form. Spectators will enjoy a guided tour of this award winning attraction, while the divers are kitted out and given their safety briefing. Spectators will then be taken into the Aquatheatre and underwater safari area to watch the divers. Spectators may have a long wait before seeing you dive after the end of the tour - it could be up to two hours - please make them aware of this. Child spectators must be accompanied by a responsible adult - they cannot be ‘looked after’ while you are diving. RESERVING YOUR DIVE To reserve your place, simply complete the booking form and send this with your booking fee as indicated. In addition to your booking fee, you must raise a minimum sponsorship of £250. You must be at least 16 years of age to take part. Please read the details on the right carefully and completely.

to raise funds for Muscular Dystrophy UK Blue Planet Aquarium Ellesmere Port, Cheshire (Off Junction 10 of M53) 2017 Registered Charity Number Registered Scottish Number SCO 39445 Shark Encounter Dive Guide Itinerary 2.00pm Arrive at Blue Planet for 2.15pm session, meet charity representative and pay in sponsorship. 2.15pm Divers go to briefing room for safety presentation. 2.15pm Divers receive safety briefing. Spectators taken on guided 1hour+ tour of award winning attraction. Sometimes tour starts at a later time to coincide with start of divers entering tank – guide will advise of start time for tour on the day 3.30pm All divers taken to quarantine area to be kitted out ready for dive 3.45pm All divers receive basic scuba training and instructions about dive 4.45pm Spectators make their way to the Aquatheatre and underwater safari in readiness for first group (there may be a wait of minutes after the tour until 1st pair of divers enter the water) 4.45pm Group 1 (2 divers) start their dive - duration 30 minutes. You do not wear flippers as you will be walking along the bottom of the giant aquarium tank. This is not a swimming activity - you are learning about the use of scuba equipment (with sharks for company!). 5.00pm Group 2 prepare for dive 5.15pm Group 1 finishes dive and Group 2 start their dive 5.30pm Group 3 prepare for dive 5.45pm Group 2 finishes dive and Group 3 start their dive 6.15pm Group 3 finishes their dive Please add on 2 hours to all times if taking part in the later 4.15pm session This itinerary is a guide only and is subject to change throughout the afternoon/evening. Spectators are advised that there may be a long wait between the first and last dive. If you are bringing young children as spectators, you are advised to make sure they have eaten before arrival as it could be a long wait before tea-time – or alternatively bring a packed tea for the spectators.
