3 What is DSPS? DSPS began providing access for students at City College in 1973. Our mission is to provide services and programs that facilitate educational access for City College Students with verified disabilities. DSPS works with students and faculty to provide appropriate and reasonable accommodations in accordance with state and federal disability laws.

5 Eligibility Requirements Who is Eligible? Students who receive DSPS services must: Be currently enrolled in classes at City College (online or on-campus) Complete a DSPS Orientation Submit verification of disability Meet with a DSPS counselor for intake appointment. Update DSPS file every semester

7 Complete DSPS Orientation Eligibility Requirements “How to…” Thoroughly read and/or listen to all of the slides in this online orientation. Complete the forms at the end of this orientation and submit them to DSPS. Complete the Orientation Verification form and submit to DSPS.

8 Submit Verification of Disability Eligibility Requirements “How to…” Examples of licensed professionals include: physicians, psychologists, audiologists, licensed clinical social workers, learning disability specialists and vocational rehabilitation counselors. (We cannot accept verification from a chiropractor or holistic health practitioner.) The student is responsible for providing documentation of his/her disability. Disability documentation must be within the last three years, and must include the diagnosis and educational limitations specific to the student. The documentation must be signed by a licensed health care professional.

9 Submit Verification of Disability Eligibility Requirements “How to…” If you do not possess verification of your disability, you can download the DSPS Medical Verification Form for your health care provider to complete. Students with Learning Disabilities -Send current documentation (including test scores) directly to City College DSPS. A counselor or LD specialist will review for eligibility.

10 Meet with a DSPS counselor for Intake Appt. Eligibility Requirements “How to…” The DSPS intake interview is the initial meeting between the student and a DSPS counselor. The intake is approximately one hour, and during that time the student and counselor will discuss topics such as: educational goals, academic history, disability information, educational limitations and possible DSPS services and accommodations. An intake appointment is required. If you live outside the San Diego area, a telephone intake appointment is possible. Call the DSPS office for an appointment after you have submitted all of your DSPS paperwork, including documentation of your disability.

11 Update DSPS file every semester Eligibility Requirements “How to…” Each semester the student must make an appointment (in person or via telephone) with his or her DSPS counselor to request appropriate services and sign a new Student Educational Contract.

13 Disability Verification Getting Started Disability documentation must be no more than three years old, and must include the specific diagnosis and educational limitations specific to the student. The documentation must be signed by a licensed health care professional. If you do not possess verification of your disability, you can download the DSPS Medical Verification Form for your health care provider to complete. Student must fill out both sides and sign the box on the top of the form. Give the form to a licensed professional to complete. The licensed professional must answer all questions thoroughly, then sign and date the bottom of the form. Form must be sent (via fax or mail) directly to City College DSPS from the office of the licensed professional verifying your disability.

15 Intake Appointment Getting Started After you have submitted all of the required forms/paperwork, you must meet with a DSPS counselor for an intake appointment. 16 Intake Appointment Getting Started During the intake appointment, your DSPS counselor will discuss: -Disability issues, including educational limitations -College policies and procedures -Individualized services and accommodations -How to arrange your online accommodations -Any other information or referrals appropriate to your needs

17 Review -Thoroughly read the entire DSPS Orientation -Complete all forms and send them to DSPS -Submit verification of your disability -Call DSPS for an intake appointment -Complete intake with DSPS counselor -Arrange Accommodations with instructor(s)
