7 Risk & Needs Prison Based Social Worker Case Co-ordinator Formal Risk Assessment File Collateral, e.g. TJR, SER Service Provider entries on CIP on PR2 Stored on PR2 Copied to Prisoner and relevant staff, including Community Based Social Worker Provides focus for Case Conference, and basis for the Action Plan Updated annually Risk & Needs Assessment Qualitative info re: è current situation è work undertaken è progress made è further work required “Distance Travelled” Ratings of need

10 Case Conferences Minimum expected attendance: Prison Based Social Worker Community Based Social Worker Case Co-ordinator Prisoner (though it continues in their absence if they decline) Clear focus on Risk & Needs Assessment Action Plan agreed with prisoner Sequencing of interventions discussed Allows for a smoother transition from custody to community Minutes confirmed with attendees, and then attached to CIP and sent to Prisoner and Community Based staff Relevant referrals made and work undertaken
