Substitute Care Provider Training May 7-15, 2012

Outcomes of Training ** After today, you will know: The difference between Respite and Substitute Care The purpose for making the change to Substitute Care When Substitute Care will begin How to arrange substitute care Background checks – How to process & review

What is the Difference Between Respite and Substitute Care?? Respite Allows opportunity for unpaid caregivers to take time off Requires an authorization from IDT Substitute Care Allows an opportunity for the paid AFH/primary provider to take a break from care giving duties Does not require an authorization from IDT May occur within the AFH or in the home of the Substitute Caregiver

Purpose – Why Are We Making This Change?? Want to continue to offer opportunity for providers to take time off from caregiving responsibilities – Avoid Burnout Request from providers to stream-line process Simplify to an all inclusive daily rate days a year reimbursement rate Positive provider response to “Budgeted In-Home Respite” policy Other MCO’s are successfully using Substitute Care

When do we start using Substitute Care?? Pro-ration of respite days began on January 1 st, All respite days must be used by June 30 th, 2012 Substitute Care will begin on July 1, 2012

How do I arrange Substitute Care?? Substitute Caregiver List?? Identify individuals that will be utilized as Substitute Caregivers Obtain signed Releases of Information for potential Substitute Caregiver Contact IDT Request “Initial Substitute Caregiver Packet/Binder” & copies of member information – Member Information Sheet, CCCW Assessment, Behavior Support Plan, (if applicable) Inform of potential Substitute Caregiver Discuss communication expectations e.g. how they want you to communicate use of Substitute Care, who will notify the guardian, etc. – Discuss any arrangements that will need to be made (i.e. transportation, day services, vocational programs, etc.)

What about the Background Checks?? Complete, Process, & Review Background Checks – DHS Chapter 12 -“Caregiver Background Checks” and Chapter 50 - “Offenses Affecting Caregiver Eligibility” – Ensure any convictions are not substantially related to the caregiving responsibilities for a member Contact Provider Network Residential Services Manager to discuss questions or concerns Colleen Seemann or Michelle Nye Provider Network Managers – Quality will review completion of background checks during AFH recertification

How is the Substitute Caregiver paid?? AFH/primary provider will receive an adjustment to their daily rate beginning on July 1, 2012, which can be utilized to pay for Substitute Caregiver AFH/primary provider will negotiate rate of pay with the Substitute Caregiver

What do I do with the Substitute Caregiver’s private information?? Create a confidential Substitute Caregiver file which contains the Substitute Caregiver’s: – Release of Information – Processed Background Checks – Copy of valid driver’s license – Copy of current auto insurance with minimum liability of $300,000

Where should I put the Member’s Information?? Binders will be provided by CCCW. AFH/primary provider will add the following information: ● Resident Medical Examination Form ● Authorization to Control/Dispense Medications ● Medication Logs ● Member Information Sheet ● CCCW Assessment ● Behavior Support Plan, (if applicable) And ● Caregiver Checklist ● Caregiver Home Checklist, if care will be provided in Substitute Caregiver’s home Note: Substitute Caregiver will utilize this traveling binder during times when they are caring for member. Binder will be returned to AFH/primary provider following visit.

What about training for the Substitute Caregiver?? Educate & train Substitute Caregiver regarding member specific service needs and record- keeping responsibilities If member needs are specialized, consider including IDT or guardian in the training Emphasize the importance of the Substitute Caregiver maintaining confidentiality
