Agenda Determination of Eligibility Student Application Individualized Education Program Reminders 06/10/20142

Determination of Eligibility Indicate if the evaluation is an Initial or a Re- Evaluation. Determination of Eligibility (DES) date is the same on – Student Application – Individualized Education Program (IEP) On the student’s IEP the date of the current eligibility certification should be the same date that is stated on the DES. 10/09/20143

Student Application All blanks and boxes need to be completed. See attached is not acceptable. Date services began are the same as the beginning date of services in the IEP. Date of the annual IEP is the same date as when the IEP meeting was held. Type of disability is the same on DES and the IEP. 10/09/20144

Student Application Placement type on student application: – Intermediate Care Facility for the Intellectual Disability (ICF-ID) Day Treatment – Intermediate Care Facility for the Intellectual Disability (ICF-ID) – Psychiatric Day Treatment – Psychiatric Residential Treatment Facility (PRTF) – Private Special School 06/10/20145

IEP Submission of an application. Make sure the names are listed on the correct side of the page. If the student has an annual IEP and a revised IEP, please submit both copies. 06/10/20146

Reminders Documents that need to be submitted with the student application: – A copy of the child’s current Determination of Eligibility – A copy of the current evaluation report – A current Individualized Education Program (IEP) developed in accordance with regulations. Remember to submit current documents as the old ones expire. 10/09/20147
