What is the Home Care Referral Registry? The HCRR is a tool that you and your clients can use to search for and select eligible home care providers. The Registry uses a combination of web-based matching logic and localized support staff that help to match clients/consumers with a pool of pre-qualified, pre-screened home care providers.

Where is the Home Care Referral Registry? Registry staff are located across the State. Web-based system used to gather and store data about prospective providers, collect information from consumers and track matching outcomes.

How does the Local Registry find Providers? Local offices build up a pool of work-ready providers the Registries by: Recruiting new potential providers through a variety of venues and activities. Reach out to current IPs to see if they would like to work for other clients or increase their hours. Potential or current providers can contact the Referral Registry to: Submit an application specifying location and assistance with tasks they are willing or trained to do. Identify their experience, availability and clientele they are contracted/willing to serve.

New Provider Enrollment To enroll onto the Registry, Providers must: complete an interview with the local HCRR contact, successfully complete a background check, have a current, signed contract (complete S & O training) Additionally, current providers must be in compliance with training and certification requirements and successfully complete fingerprinting within required timeframe.

Case Managers Case Managers, eligible consumers, families or designated representatives can contact the local Registry Coordinator to request a matching provider list of eligible providers. Registry Coordinators: Gather information about the consumers location, language, service needs and preferred schedule. Confirm eligibility with the case manager. Create a best match list Verify provider eligibility and availability Send the list to the appropriate contact and follow-up to offer assistance or a new list.

Matching Provider List Confirms eligibility of potential providers that appear on the referral list. Pre-contacts potential providers to verify availability and notify of contact from prospective consumer. Provides the referral list to client, family, designated Rep or requesting CM. The Registry Coordinator generates a Matching Provider List when a request is received and consumer information is gathered. The List identifies screened and eligible providers, based on best-match criteria. Once the list is generated the Coordinator will: SAMPLE

After Provider is Selected The HCRR Coordinator will: Confirm with the provider that he/she was selected and is willing to work for the client. Contact the case/contract manager to identify the chosen provider and transfer the provider file and other required documentation to the appropriate contact. Follow-up with the consumer/family or designated representative to assure the chosen provider is working out and offer further assistance if needed. Authorizing HCRR-based IP’s

Authorizing New HCRR-based IP’s When new IPs are contracted through the Home Care Referral Registry (HCRR), they will complete their required 2 hours of orientation and 3 hours of safety training through the HCRR, but will not be paid by HCS/AAA/DDD staff until they are matched with a client who hires them. This training must be added by HCS/AAA/DDD staff to the Training Partnership database. IPs are paid for training time once they are associated with a client. Fingerprint Requirements: Local Registry staff schedule appointments for new providers to have prints taken at nearest MorphoTrust location upon their initial match with a client. The appointment form and/or the corresponding receipt from MorphoTrust that prints were rolled will be included in the provider file. Review the form with a client when he/she initially chooses to hire an IP. The client or representative must sign and date the form. Document the client’s provisional hiring decision for each IP hired in a Service Episode Record (SER).

Provider Recruitment & Support HCRR staff focus on active recruitment in their communities in order to build pool of eligible providers. Conduct screening and initial contracting. Provide assistance to new providers throughout screening, training and certification process.

Access to a local pool of screened and pre-qualified home care providers. A “best match” based on client and provider identified needs and preferences. Information and education on hiring and supervising home care providers. Providers for emergent and short term needs. The Registry offers client’s choice and control over their services and supports:
